IL Podiatrist Discusses Heel Bursitis

If you have bursitis, it often feels like a bump, usually on the back of a heel. “This condition can occur when you have a genetically enlarged heel bone,” says Dr. Marlene Reid, a podiatric surgeon in Naperville, Illinois.

Dr. Marlene Reid

“It’s often called ‘pump bump’ because the bone can become irritated from wearing a shoe like a pump with a stiff back,” says Reid. When the unforgiving shoe rubs against the bone, you can inflame the bursa, the protective sac of fluid in the tissue. If the problem causes chronic pain, a podiatric surgeon can remove the bursa and the enlarged part of the heel bone.

SUCCESS TIPS FROM THE MASTERS

Editor's Note: PM News is proud to present excerpts from Meet the Masters.

Bret Ribotsky: How will residencies be affected under the future Obama Healthcare program?

Dr. Edwin Wolf

Edwin Wolf:  On one hand, we know that the federal government wants to cut spending, and healthcare is clearly a large part of it.   Medical education has not been focused on with the funding cuts. It may be that Obama wants healthcare for everyone, and that will require more doctors, and these doctors need to be trained.

Dr. Orlando Mercado

Meet the Masters is broadcast each Tuesday night at 9 PM (EST). This week's guest is PM Podiatry Hall of Famer Dr. Orlando A. Mercado. You can register for future events by clicking here

RE: Efficacy of Lasers for Onychomycosis (Burton Katzen, DPM)
From: Burton Katzen, DPM

In response to the storm of e-mails I have received (many from laser companies and collegues associated with laser companies), I never said that lasers were not one of several effective methods for treating onychomycosis. I’m sure they would have not received FDA approval if they weren’t. However, that being said, I was never promoted a laser by a company showing me copies of pre- and post-laser lab reports, only pre- and post-laser nail photographs.

Also, in my opinion, patients would not be overly impressed with negative post-laser cultures after spending the money if their nails are still thick and “ugly.” Whether the companies or my colleagues care to agree, the patient’s perception is that they will come in with an ugly nail and leave with a pretty nail. My only point was that I believe it would be unethical not to tell patients that this may not be true in the majority of cases. Once again, my only warning to colleagues contemplating purchasing a laser is to not promise your patients with thick nails “beautiful nails” in six months.

In my experience, it’s not going to happen, and both of you will be disappointed. It was frustrating enough in the past explaining to patients that if you take Lamisil pills, all the “little fungus people” won’t run out in horror from underneath your nails.

RE: Efficacy of Laser for Onychomycosis (Tip Sullivan, DPM)
From: Keith D. Sklar, DPM

I’d like to share my experience on the use of lasers with nail fungus. I use my laser as one segment of my treatment protocol to clear and maintain a healthier nail plate. I also address the surrounding skin. I have found that to take a dystrophic nail plate and convert it to a healthy nail plate is a long-term process. I inform all my patients that they must be very patient and compliant. We discuss a custom pathway and treatment plan that will address their condition, providing the patient with the most optimal results available.

In moderate to severe cases, my treatment consists of the use of the laser and topical medications. We do employ an oral medication. Once the treatment plan has been made, the process of...

RE: Efficacy of Lasers for Onychomycosis
From: Dale Feinberg, DPM, CPed

The conversation here is really between efficacy and reality. If one performs a bunionectomy with less than desirable results, you do not refund your fees. If you invest in the most advanced ultrasound technology and get reimbursed at 20% of the golden day fees, you do not complain. You have invested in and practice retail medicine, yet accept wholesale reimbursements.

You are a pawn in the game. Imagine bringing home a shiny new BMW and telling the dealer to bill some random car payment company who randomly decides if, when, and how much to reimburse the dealership. When I turn on my Cool Breeze laser several times a day, I have already been paid MY pre-determined professional fee and feel like the king of the world. So, if there's something wrong in the neighborhood, who ya gonna call- "Fungus Busters."

RESPONSES / COMMENTS (NEWS STORIES)

RE: Gravesite of Lincoln's Foot Doctor Now a Tourist Attraction
From: Richard A. Hacker, DPM

Dr. Isachar Zacharie was more than President Lincoln's chiropodist. He was one of Lincoln's closest confidantes and a recognized Union secret agent. He was directly asked to have provided vital information on Confederate troop movements during the Civil War in 1862. His efforts for Lincoln and our country had escalated, eventually extending into diplomacy. His influence eventually became the critical push to ending the Civil War.

The following year, he reported to President Lincoln and Secretary of State, William Seward that he had met with the Confederate Secretary of State and other of their cabinet officers. He is credited with brokering the initial negotiation attempt for peace talks between the leaders, and was eventually rejected by the Congressional leaders. This was highlighted in the recent Hollywood movie, but without mention of the doctor. Zacharie was appointed Chiropodist-General to the United States Army. Unfortunately, his services were not properly recognized or reimbursed after Lincoln's death, and he eventually chose to return to England. Interestingly, he is cited as bringing and starting an order of the Freemason from the U.S .to England. He remained there until his death. Further information is readily available on this great Jewish American and chiropodist.
