The Academy of Continuing Podiatric Medical Education (ACPME) held a two-day virtual seminar last weekend. The seminar which drew a near record attendance of over 375 podiatrists was organized by ACPME's scientific director Robert Marcus, DPM and Director of Continuing Medical Education David George, DPM.

Metabolic Stress May Explain Dementia Risk in Diabetes
A retrospective analysis of over 268,000 U.S. adults aged 50 years or older reveals that non-obese individuals with diabetes have a higher risk of developing dementia compared to those without diabetes. The research suggests metabolic stress, not obesity, drives this risk. Type 1 diabetes without obesity showed a fourfold increased risk for dementia, while type 2 diabetes was associated with an increased risk by 82%. Patients with obesity and either type of diabetes showed a lower risk for dementia. These findings highlight the need for proactive cognitive screening and management in non-obese diabetic patients. No funding was reported.
The Takeaway: Non-obese patients with diabetes face increased risk for dementia and should receive proactive cognitive screening and metabolic optimization.

While this story is done with a humorous tone, it is nevertheless a very sad commentary on the entire state of the insurance industry and can be extended to all health and other types of insurance offerings. Holding dinners and luncheons at restaurants, providing free food, alluring patients with other bonuses such as "free dental" and transportation to/from doctors' offices are but a few of the lures these plans use on the unsuspecting.
Free dental? Really? Until they read the full prospectus which those who do, will run from. They barely provide cleaning and x-rays from a limited number of dentists. Other than that, these unsuspecting patients find out they will be paying out-of-pocket for most dental work, just as they would in traditional Medicare. The dentists who do participate in these plans usually are not the patients' dentists, nor are the listings accurate. And what about the other promises and...

I'm sorry to report the passing of Kevin Moran, DPM of Westminster, MA on 11/3/2025. Kevin was a professional colleague, friend, devoted family man, and an outstanding podiatrist. Most of all a true gentleman, and someone to be admired.
A graduate of Assumption College, ICPM, he was active in professional practice for 32 years prior to retirement from Fallon Clinic in Worcester MA. Kevin was a member of the APMA, and the Massachusetts Podiatric Medical Society serving as its president in 1992. Kevin also was a Diplomate of the American Board of Foot and Ankle Surgery.

I smashed my left pinky toe into a door frame. It hurt like crazy. I went and got it x-rayed, and sure enough, it was fractured. A month later, at a routine podiatrist visit, my toe was still swollen and purple, and my toenail was black. I explained to the doctor that I had fractured it. He looked closely at my mangled little piggy and said, “Yeah, you’re going to lose that toe.
"What?!" In about three seconds, I went through the shock, acceptance, and determination of living without a toe when the doctor said... “Sorry, I mean toenail. You’re going to lose that toenail.”
