PA Podiatrist Treats Bite from Poisonous Caterpillar

A woman who came into contact with a poisonous hickory tussock caterpillar earlier this summer wants to warn others, especially curious children, before they suffer the same fate. After stepping on a caterpillar, she was initially mis-diagnosed with gout at an urgent care center and then with a stress fracture at her PCP who referred her to David Pinegar, DPM, a podiatrist. During a follow-up appointment with Dr. Pinegar, 27 days after coming into contact with the caterpillar, he took one look at her foot and said “this is no stress fracture.”

Dr. David Pinegar and hickory tussock caterpillar

Dr. Pinegar believed it to be an infection from a bug bite and ordered blood work. The blood tests did not show any infection, but indicated above-normal antinuclear antibodies. She was prescribed steroids which reduced the swelling and pain level down to 1, she said. The pain and swelling returned the day her meds ended. Dr. Pinegar prescribed meloxicam, which is used to treat rheumatoid arthritis. After 35 days, her symptoms went away and did not return.

New Hammertoe Fusion Device Now Available For Licensing

The device is made with an innovative shape memory material, which helps provide better compression and stability than comparable devices. MACRA UPDATE
After Delay, CMS Says Physicians can Expect Advanced APM Bonuses Soon

After an appeal from nine physician organizations, the Centers for Medicare and Medicaid Services (CMS) says doctors and other healthcare professionals who qualify for an advanced alternative payment model (APM) performance bonus should get their money soon.

CMS announced in a fact sheet that incentive payments will go out soon to clinicians who earned a performance bonus as part of their participation in advanced APMs in 2017. CMS said it would begin paying the incentive payments, due for the first year of its Quality Payment Program, starting this month.

PRACTICE MANAGEMENT TIP OF THE DAY
How to Have a Well-run Practice - Part 8

The daily grind of seeing patients can sometimes be, well, grinding. It’s important to take a step back and reconsider how you do things. There’s likely another way to achieve the same, if not better, result, especially when it comes to staffing. Here’s a sample of what 943 practice owners, managers and physicians say has helped them create a better environment.

8Equip employees with the tools they need to be successful in their role. Provide comprehensive onboarding to prepare new staff for their new role. Consider quarterly training for all employees, especially with new requirements and insurance demands.

CODINGLINE CORNER
Query: Repair of Achilles Tendon Intra-substance Tear

What is the recommended coding for repair of Achilles tendon? This was an intra-substance tear. Would CPT 27650 require a complete rupture repair?

Lawrence Kassan, DPM, Philadelphia, PA

Response: CPT 27650: Repair, primary, open or percutaneous, ruptured Achilles tendon
CPT 27652: Repair, primary, open or percutaneous, ruptured Achilles tendon; with graft (includes obtaining graft)
CPT 27654: Repair, secondary, Achillies tendon, with or without graft

If the Achilles tendon suffered an “intra-substance tear,” the most appropriate CPT code to use for the procedure to repair the Achilles tendon would be CPT code 27654.

RESPONSES/COMMENTS (PM NEWS QUICK POLLS)
From: Robert Scott Steinberg, DPM

I wonder if those 16.35% of podiatrists who are not going to get a flu shot will post caution signs for their patients, telling them, while they are doctors, they don't believe in medicine. FYI, my hospital requires all physicians to get flu shots, which they can get at the hospital.
