PRACTICE MANAGEMENT TIP OF THE DAY |
Six Back Office Billing Strategies
2) Reconcile encounter forms and bill claims daily.
• If there are any questions about the services rendered, seek clarification from the provider. Your billers should be knowledgeable about appropriate modifiers and when to use them.
• Make sure your billers are submitting clean claims. Clean claims get paid the first time around and stand up to a potential audit.

QUERIES (NON-CLINICAL) |
Query: Scanning Paper Charts
I am preparing for eventual retirement and need to scan paper patient charts to facilitate storage of patient records for the mandatory 7-year period of time. I plan to use a multipage feeder desk top scanner and external drive for storage and easy transport. It will probably take my staff a year to accomplish this by transferring charts between patients during office hours. Any better ideas would be appreciated. I am an old dog and not computer savvy. Hence, the paper charts.

RESPONSES/COMMENTS (CLINICAL) |
From: Stanley Beekman, DPM
In the rare cases of plantar fasciitis which were non-responsive to conservative care, I would get an MRI which would show: bone marrow edema, mucoid degeneration of the plantar fascia, and partial tearing. The procedure I would use removes the calcaneal spur to treat the bone marrow edema, then I would inject the platelet gel (spun down at the hospital) into the plantar fascia.
The initial mistake I made was not immobilizing the patient post-op. I found that 3 days off-weight-bearing and then the use of a CAM walker for a month was required.

RESPONSES/COMMENTS (NON-CLINICAL) |
RE: Proper Blade for Debridement of Calluses
From: Ivar E. Roth DPM, MPH
I called my son, a third-year student at Temple, and asked him about his clinic experience with routine care. I was shocked at his answer. He said that it is okay and standard to use a disposable dermal curette for corns and calluses. I asked him if the clinicians were okay with this, and he said yes. He told me that using blades were too unpredictable and they wobbled and caused cuts. I am sending him a #20 handle and some #20 carbon Rib-Back blades which do not flex so he can properly do the job.
Now I can understand why the newer practitioners are shying away from C and C. If this is the new standard of care now, I understand what is going on. I think we need to make sure that the schools are teaching our students proper protocol and using the proper instruments for routine care. There is nothing like a #20 Rib-Back to do a GREAT job on calluses.

RESPONSES/COMMENTS (OBITUARIES) - PART 1 A |
From: Harold W. Vogler, DPM
I am so very disappointed to learn of Gary's death. He was one of my surgical residents dating back to 1978 at Harrison Community Hospital in Michigan. He was recognized as something "special" even then. He had a keen analytical mind and questioned just about everything, but in a positive academic manner. Ultimately, I recruited him to the Dept of Surgery at PCPM mid-1980s. He quickly became a favorite of the students and staff. He demonstrated an affinity to teach students and residents, with a mild manner and style that was unique. As a result, he was awarded "professor of the year" several times.
His precise surgical style in the OR was measured, calm and controlled, and allowed an exceptional learning experience for students and residents alike. He spent hours in the dissection laboratory detailing and photographing almost all surgical procedures, to facilitate teaching. I think the entire surgical department learned much from Gary. I often quietly entered his classroom while he was in action to watch and learn from him.
Gary's personal and professional demeanor was also something special--understated and engaging for sure, but precise all the time. His life consisted of the study of podiatric surgery, sailing, and the animals of his "farm". We became the closest of friends in and out of the OR. He was one of my most trusted confidants, always providing me sage guidance on a variety of matters. I am happy that I have many incredible stories and memories of Gary. He made me a better surgeon. I will miss him. My condolences to his family. RIP.
Harold W. Vogler, DPM, Sarasota, FL

RESPONSES/COMMENTS (OBITUARIES) - PART 1 B |
From: Elliot Udell, DPM
Gary and I knew each other well. We both lectured for many years at the No Nonsense Podiatric Seminar in Cleveland, Ohio. Gary was a full-time professor of surgery and lectured on the latest surgical procedures. He was also a student and protégé of Dr. Harold Vogler who also was a professor at Temple, and he would often quote Dr. Vogler's extensive surgical research in his lectures.
Gary had a great sense of humor and was a real people person. He would spend a great deal of time at the seminar, after his lecture, being with the other participants. You got the feeling when you spoke with Gary that he listened to you, and that form of empathy is admirable. We lost a good person and may his memory be a blessing to all who knew him.
Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS (PM NEW QUICK POLLS) |
From: Robert G. Smith DPM, MSc, RPh
I would like to thank PM News for posting this survey question as well as the 373 respondents. Perhaps APMA will have an opportunity to address this question at their House of Delegates meeting if/when a resolution with this central theme is presented. I appreciate the fact that a change in curriculum may increase the cost for the program.
Given there is no national oversight for the opioid settlements, there is state oversight. My research has revealed that funds will be set aside for medical post-graduate training for safe opioid prescribing, monitoring, and detection of substance use disorder in some states. I believe our podiatric profession should share in these funds. I have written to each attorney general, acting as the founder/president of my educational non-profit (501(c)(3), inquiring about an accounting of opioid settlement distribution.
