Less Swelling and Pain with MIS Bunion Surgery: TN Orthopedist
 
Benjamin Grear, MD has done bunion surgeries for years at Campbell Clinic. The ones he’s doing lately mark a new era. Instead of a long cut on the side of the foot, he makes four tiny incisions, stitching them up at the end with a stitch apiece instead of the 8-10 stitches the traditional bunion surgery requires. "The biggest thing is it allows for smaller incisions, and that really helps the soft tissue recovery," Dr. Grear said. "There’s less swelling and less pain."

With the less invasive surgery, Grear uses a 2-millimeter burr to drill tiny holes in the foot, cuts and realigns the metatarsal – and sometimes the bone in the big toe – and then sets the new alignment with screws. The minimally invasive surgery is done under sedation and local anesthetic at Campbell, not general anesthesia. It is covered by insurance.

CA Podiatrist Granted “MD Affiliate” Status To AOSSM
 
Recognized as one of podiatric sports medicine’s most experienced practitioners, Howard F. Liebeskind, DPM was recently granted “MD-Affiliate” status into the nearly 4,000 member American Orthopaedic Society for Sports Medicine (AOSSM). He is the first podiatric sports medicine specialist to be so honored.

Dr. Liebeskind currently serves as team podiatrist to the Los Angeles Lakers, Los Angeles Galaxy, Los Angeles Football Club, and the U.S. Men’s and Women’s National Soccer Teams. Dr. Liebeskind is a Fellow of the American Academy of Podiatric Sports Medicine and is Board Certified in Sports Medicine by the American Board of Multiple Specialties in Podiatry.

CT Podiatrist Discusses Shin Splints
 
"One of the most common injuries is shin splints, especially among beginner athletes. The term shin splint is commonly used to describe pain in the lower leg. The most common sports associated with shin splints are those sports such as jumping, or those that have a change to a hard playing surface such as football, soccer, or aerobic dance. The condition, however, can occur in any sport and is most commonly found in the unconditioned, untrained athlete," says Robert Weiss, DPM.

One of the major problems causing the overuse injury is pronation of the foot. It is most commonly seen in people who have flat feet or low arches. The stretching of the foot inward places increased pull on the muscles that run along the inner border of the ankle and under the arch. These muscles originate along the inner side of the lower leg. Treatment of shin splints involves muscle strengthening of the shin muscle and changing the cause and effect of the pronation-flatting of the foot on the leg," says Dr. Weiss.

RELEVANT RESEARCH
Wart Removal Without Anesthesia Using Long-Pulse 1064-nm Nd:YAG Laser
 
This retrospective chart analysis included 85 patients with a total of 174 warts treated with ND:YAG (1064 nm) without topical or infiltrative anesthesia over the course of a nearly 3-year period. Most warts (66%) were on the plantar surface. All warts were treated with about 10 sequences of 3 to 4 pulses with an overlapping circle technique aimed at minimizing damage to surrounding tissue and maximizing energy delivered to the wart. Cooling with ice and cold air was used between treatment sequences. In 68 patients, full clearance was attained in a mean of 2.2 sessions, and side-effects were minimal. Pain level during the procedure was a mean of 6 on a visual analog pain scale. Seventeen patients did not complete treatment, but only 4 listed pain as the main reason for discontinuation.
 
The 1064-nm Nd:YAG laser without anesthesia was well tolerated with very few patients discontinuing therapy due to pain; additionally, it was effective, with complete clearance in 68 of 85 patients.

Query: Impossible Injection
 
I am having trouble billing/getting paid by Anthem Blue Cross for a dexamethasone phosphate injection. I have tried several ways to bill the NDC number, but I always get a denial saying it is missing, invalid, or termed. Further, because of that “error” they do not pay any other lines of service for the same DOS. Other insurances do not deny me. The NDC# on the bottle is 55150-239-30 for dexamethasone sodium phosphate injection, 120 mg per 30 mL (4mg/mL). I have researched many websites for help. The last time I billed, the NDC info was placed on the supplemental line for J1100 as N455150023930 ML30 10 00. I normally inject 2 mL (8 mg) and charge for 1 DOU. What am I doing wrong?

Response: The only thing I can see about the information you placed on a claim about the dexamathasone might be the format of the NDC number. The format is xxxxx-xxxx-xx. There are 5 digits in the first section, 4 in the second section, and 2 in the last section. If there isn't that amount on the bottle, you should place zeros before the number to make it match the 5-4-2 format. Try that and see if it works. If you are still having an issue, I would remove the injectable and bill the claim without it.

RE: The Passing of David Y.S. Lee, DPM
 
The Yee and Tashima families are deeply saddened to announce the passing of their beloved son, brother, and husband, Dr. David Y.S. Yee passed away peacefully at his home on Wednesday, August 26, 2020 in Honolulu, Hawaii. He was 58.
 
Dr. Yee graduated from the California College of Podiatric Medicine. Upon returning to Honolulu, he joined the family podiatric practice, Hawaii Foot Clinic in downtown Honolulu, working beside his mother, Dr. Helena Yee. He eventually opened his own practice, Honolulu Foot Care.

QUERIES (NON-CLINICAL)
Query: Light Orthotics
 
I am looking at a system from Powerstep called "Light Orthotics" that uses a machine and a proprietary flexible shell that Powerstep manufactures. The result is a custom orthotic molded directly to the patient's foot. I do not think this device is as good as a lab-fabricated orthotic, but it does seem to be a quality device that would help patients with some biomechanical abnormalities. Does anyone use this system? If so, what are your thoughts?

If the parent is concerned enough to seek out medical treatment for the 2 year old with an elevated 2nd toe, or any other age child with a foot deformity, it is incumbent for a podiatrist, who hopefully specializes in the care of pediatric patients and has experience in treating these conditions, to give the parent the treatment options available, surgical vs. non-surgical and the prognosis as well. For pediatric congenital curled toes, elevated toes, and flexible hammertoes, I often offer non-surgical modalities, including daily home stretching and taping 24/7.
 
The results are dependent on many variables, including but not limited to...

According to the UnitedHealthcare website, waived co-pays must be refunded to patients. It seems unfair and perhaps illegal not to compensate doctors for unilateral decisions in policy. I will raise my fee for providing patient records for chart reviews in response. Any lawyers out there for a class action suit? LOL.
