PODIATRISTS AND SPORTS MEDICINE
CT Podiatrist Discusses Sever's Disease

One of the most common soccer injuries we have seen is the large number of young soccer players complaining of heel pain. This problem is often caused by a condition known as apophysitis of the calcaneus (heel bone) or Sever's disease. "This condition consists of an inflammation of the growth area (heel plate) of the heel bone, which has not completely matured or closed together, and has developed in two parts. It is most commonly seen in boys and girls between the ages of 10-15, says podiatrist Dr. Robert Weiss.

Dr. Robert Weiss

"The pain is usually present in the back of the heel and is more pronounced in running and jumping sports. Sometimes, there is a limp in the child's gait, of which he may not even be aware. It has been described as an osteochondrosis, which is a loss of blood supply to the apophysis," says Dr. Weiss.

PODIATRISTS AND FOOTWEAR
Wearing High Heels Shortens Achilles Tendon: PA Podiatrist

Your Achilles tendon, or the band of tissue stretching from the back of your heel all the way to your calf, lengthens and shortens with your foot movement. When you point your toes and your heel rises closer to your calf (like you do when wearing heels), your Achilles tendon shortens. But too much shortening is a bad thing, according to Dr. Jane Pontious.

Dr. Jane Pontious

Because the Achilles connects to the plantar fascia, or the ligament that stretches along the bottom of your foot, shortening your Achilles tendon pulls on the plantar fascia, causing it stress. The result is a form of pain in your heel and arch known as plantar fasciitis.

OUTSIDE INTERESTS
CA Podiatrist Defines What a Mensch is

Two weeks ago, in a flash of the kind of kismet that seems to pervade Dodger Stadium these days, Phil Wojdak, a blind fan, stepped on another fan’s food as he bumped his way to his seat. This being Dodger Stadium, the fan with the smashed food could’ve been anyone — a thug, a mogul, a preacher, a movie star. In a grand piece of luck, the fan turned out to be Dr. Michael Levi, a podiatrist who works with local teams, volunteers everywhere, even donates his free time to clip orangutan toenails at the L.A. Zoo. In short, Levi is a mensch of all mensches.
Dr. Michael Levi

When Levi, who like any 57-year-old kid, brought his Rawlings glove to the game, snagged a foul ball off a pitch thrown by Kenta Maeda, he quickly came up with a plan. A season-ticket holder, he sent the ball to the Dodgers, and a week later treated the Wojdaks to dinner, where he presented them with a token of his new friendship — the ball thrown by Maeda, with the ballplayer’s autograph.

PRACTICE MANAGEMENT TIP OF THE DAY
Four Ways Doctors Can Take Advantage of Long Weekends- Part 4

Approached correctly, long weekends can be a time for enjoyable leisure, rest, and relaxation. Approached like a typical weekend, they might offer no significant benefit. How can we optimize these calendar gifts and still maintain our equanimity? Here are several ways to do so:

4) On the last day of your long weekend, be it a Sunday evening, or in some cases a Monday evening, don't overdo it. Don't stay up late, overeat, overdrink, or borrow against the next day. Retire at a reasonable hour, and be prepared to head back into work a little earlier than usual — to encounter less traffic — and provide yourself a few extra minutes back in the work-a-day world.

QUERIES (CLINICAL)
Query: Dr. Win's Fungal Nail Therapy

I'm curious about the results from those using Dr. Win's fungal topical? I assume this topical is for mild to moderate conditions, as are most of the other topicals.

QUERIES (EMR)
Query: EHR Registry Exemption

Since Stage II MU for 2016 requires that podiatrists report through a specialty registry for treatment of flatfoot (adult and congenital) and for plantar fasciitis, then is it true that you can be "exempt" for this measure if your vendor is not able to download this electronic information to this specialty registry?

Here is what I received from my EHR software vendor today on this exact issue regarding the exemption"Since your vendor is unable to create the type of file we need to import for our registry, you will be able to claim the alternate exemption for the specialty registry measure for meaningful use. Here is the information:" Can someone provide clarity on this issue?

QUERIES (NON-CLINICAL)
Query: Source for Size 17 Post-op Shoe

I have a patient who will require a size 17 post-op boot. Does anyone know a manufacturer that makes a post-op tall boot that large, or am I going to have to cast this guy?

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Perform a Semi-Annual Review
From: Hal Ornstein, DPM

Twice a year, sit with each staff to have a simple exchange. Here’s what to ask:
1. What ideas do you have to improve the office?
2. How can we make your life better in the office?
3. How I can be a better boss?

A week after the session is complete, have an office meeting to discuss ideas from the one-on-one meetings.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2

I am also interested to know if the ABFAS will develop some type of senior or honorary designation. I was initially certified by ABPS in 1993 and recertified in 2003. I did not recertify in 2013 because I was working part-time and not doing surgery; therefore, I was not on staff at the hospital or surgery center. I have now been in practice for 30 years and will continue (because of necessity) practicing. Due to losing by active certification, I was discontinued from my local physicians group that certifies one for insurance company membership.

My intelligence/competence has not changed. I continue to keep updated. I know some of the Board members think that if you are not actively doing surgery, you cannot continue being certified. However, we did earn this title and there should be some certificate regarding this. I know my PHO would have accepted a letter or honorary certificate from the ABFAS so that I could maintain my insurance status. I am sure there are others in my position. Am I the last solo practitioner who wants to maintain a livelihood in all respects?
