Wearing Shoes Too Soon Can Hamper a Child's Walking and Cerebral Development: UK Podiatrist

Tracy Byrne, a podiatrist specializing in podopediatrics, believes that wearing shoes at too young an age can hamper a child's walking and cerebral development. "Toddlers keep their heads up more when they are walking barefoot," she says. "The feedback they get from the ground means there is less need to look down, which is what puts them off balance and causes them to fall down."

Tracy Byrne

Walking barefoot, she continues, develops the muscles and ligaments of the foot, increases the strength of the foot's arch, improves proprioception (our awareness of where we are in relation to the space around us), and contributes to good posture

Medicare Pay-for-Performance Plan Criticized Over Early Launch

Members of organized medicine are sharply critical of a plan by the Obama administration to initiate a Medicare value-based purchasing program two years before federal law requires it. Although the 2015 start date of pay-for-performance is mandated by Congress, CMS plans to use a 2013 reporting period to determine how pay will be adjusted for some physicians in 2015.

The health system reform law requires the Centers for Medicare & Medicaid Services to use a value-based payment modifier -- another term for pay-for-performance -- for some physicians starting in 2015. The modifier would adjust payments to physicians based on the quality of care they provide and how much cost they incur relative to their peers during the course of a reporting period, CMS said. All physicians in the program would be subject to the modifier starting in 2017.

SUCCESS TIPS FROM THE MASTERS

Editor's Note: PM News is proud to present excerpts from Meet the Masters.

Bret Ribotsky: If podiatrists acquire MD or DO degrees, do we risk losing our identity?

Dr. Barry Block

Block: No. We were podiatrists when we were PODs and DSCs. We have been DPMs now for the last 50 years, but that’s not what defines us. The word podiatrist is what defines us and I think the general public has a good idea of what we are as opposed to when I first entered the profession in 1976. Then, people occasionally used to say, “Oh, you’re a chiropodist.”  The general public and medical community now know that we are foot doctors. Does that mean we can’t educate them more? Obviously, we need to keep educating them.

Dr. Lee C. Rogers

Meet the Masters is broadcast each Tuesday Night at 9 PM (EST). This week's guest is Congressional candidate and wound care specialist Dr. Lee Rogers. You can register for future events by clicking here

PRACTICE MANAGEMENT TIP OF THE DAY

Don’t Say ‘Yes’ All the Time

To protect your time, don’t let “Yes” be your default answer when someone asks you to do something. Put all requests to this test:

  •  “Should someone else do this?” The task might require someone with different skills, or it might not require your level of expertise. Suggest a colleague or delegate the task if you aren’t the best person for the job.
  •  “Can I do this another time?” Be helpful while controlling your schedule. Example: “I’d love to serve on the committee next quarter, but right now I need to focus on another project.”
  •  What information do I need?” Before you commit to doing something, clarify the request. Example: Ask to see an agenda before you agree to attend a meeting.

QUERIES (MEDICAL / LEGAL))

Query: Waiver from Suing

Are doctors allowed to place in their consent forms waivers that patients cannot sue?

Jeffrey Kass, DPM Forest Hills, NY

Editor' comment: PM News does not provide legal advice: Most courts would deem such a waiver as "against public policy." Additionally, many patients would not consent to surgery if they had to sign such a waiver.

QUERIES (NON-CLINICAL)

Query: Source for In-shoe Heel Lifts?

Where is the best place to purchase 1/2 inch heel lifts to fit into a patient's shoe?

RESPONSES / COMMENTS (JURY VERDICT REPORTER)

RE: Excessive Removal of Bone (NY) $1.5 million
From: Daniel Chaskin, DPM

The plaintiff's expert podiatrist testified that the metatarsal was positioned downward at an almost 40 degree angle. She also testified that 1 centimeter of bone had been removed, which equaled one sixth of the first ray. Can this be appealed?

Daniel Chaskin, DPM, Ridgewood, NY, podiatrist12@verizon.net

Editor's comment: PM News does not provide legal advice. In general, only the amount of a judgment can be successfully appealed. Opinions of medical experts can be challenged on cross-examination, but ultimately the jury must decide how much weight such statements have.

RESPONSES/COMMENTS (NEWS STORIES )

RE: APMA Nixes MD/DO Referendum
From: Ivar Roth, DPM, MPH

Until recently, I thought having an MD degree behind our names was a good move for the profession. Since my original knee-jerk reaction, I have changed my mind. Here is why. We have a great profession which we should be proud of. We are a distinct specialty area that is unique, much like the dentists have their area of specialization. With our recent educational and residency upgrades, we have finally arrived and the answer is not to group us in with an MD degree where we could lose our uniqueness. Podiatry should do what the state of California is doing.

California is instituting, in the near future, a physicians and surgeons status for us which will give us an unlimited degree to do whatever we have been training for and does away with all the credentialing issues and turf battles. This strategic change allows us to keep our degree but now practice to whatever level we are competent at. The process will require changes in the curriculum at our podiatry schools so new graduates will be P&S certified and equivalent to their MD counterparts. A pathway for those already in practice to upgrade themselves to P&S status will be made available, thus solving those issues. This solves everyone’s problems and is a way to truly get parity but keep our uniqueness intact. America and the world needs DPMs. We have the potential to have unlimited growth in the short and long-term as our services are needed and growing every day with the aging population.

Ivar E. Roth DPM, MPH, Newport Beach, CA, ifabs@earthlink.net

Editor's comment: The analogy of podiatry to dentistry would only hold true is the medical specialities of dermatalogy and orthopedic surgery did not exist. The world needs podiatrists and podiatrists need equal payment and treatment.
