INTERNATIONAL PODIATRISTS IN THE NEWS

UK Podiatrist Climbs Volcano for Charity

A podiatrist who lives near Pershore is putting his best foot forward to raise funds for charity. Jonathan Small sets off today on an ice-climbing trek across one of the highest active volcanos in the world. He’s climbing Cotopaxi in Ecuador to raise money for the Raynaud’s and Scleroderma Association in memory of his mother Pat Small, from Walsall, who died from the disease 16 years ago.

Jonathan Small

Jonathan was all set for a similar challenge last year, but it was beset with problems. This year, an old injury threatened to scupper his plans, but after knee surgery in March, and lots of exercise ever since, Jonathan declared himself fit and well. The father of one has already raised nearly £4,000 and is keen to increase that total.

INFECTIOUS DISEASE NEWS

Antibiotic Ointments May Fuel Resistance and Spread of MRSA

Bacitracin and neomycin, two common ingredients in over-the-counter antibacterial ointments, may be selecting for a specific clone of methicillin-resistant Staphylococcus aureus, according to a study published online September 14 in Emerging Infectious Diseases. There is an increasing worldwide prevalence of community-acquired methicillin-resistant S aureus (CA-MRSA), and one clone - MRSA USA300- appears to be spreading rapidly in the United States.

Community-acquired soft tissue infections are commonly treated using over-the-counter ointments known as triple-antibiotic ointments (TAOs), including Johnson & Johnson's Neosporin (polymyxin [PL] B sulfate, bacitracin, and neomycin) and Polysporin triple ointment (PL B sulfate, bacitracin, and gramicidin). The antibiotic components of these ointments are present in concentrations much higher than their minimum inhibitory concentrations (MICs). The study suggests that bacitracin and neomycin resistance in response to TAOs may be driving the spread of USA300 in the United States. In Japan, TAOs are not commonly used, and so there is little selective pressure for bacitracin and neomycin resistance.

PRACTICE MANAGEMENT TIP OF THE DAY

Capture What’s Going Right

Things really aren’t as bad as they seem. The problem is that most people remember negative events more than positive ones. That leads to reacting to difficult situations rather than approaching them in a calm and thoughtful manner.

Train yourself to find the value in any situation. Answer these questions:

  •  “What’s working?” Instead of dwelling on what went wrong, think about the many things that went well. Example: You missed your flight to a CME seminar, but discovered you could take the courses on-line.
  •  “How can I use this to my advantage?” That’s right: Make lemonade. Example: If a patient cancels an appointment with you at the last minute, how can you best use that unexpected available time?
  •  “What went well today?” Make a daily practice of writing down three good things that happened, no matter how small. Soon you will become more attentive to the positive events in your life.

RESPONSES / COMMENTS (NON-CLINICAL)

RE: Professional Attire (Carl Solomon, DPM)
From: Paul Busman, DPM, RN

Wearing scrubs outside the clinical setting defeats their whole purpose. The reason you wear scrubs in the OR is to prevent outside bacteria from entering the OR. If you wear them on the street, that benefit is gone. As Dr. Solomon noted, wearing those scrubs outside after surgery risks bringing infectious organisms outside.  For the OR, at least, keep scrubs where they belong.

If you're just wearing scrubs as a professional "costume" and for comfort in the office, this is less of an actual problem. Still, the public may see those scrubs and think of bloody surgery and be put off.

RESPONSES / COMMENTS (NEWS STORIES)

RE: APMA Nixes MD/DO Referendum
From: Kevin Lam, DPM

In regard to the debate about MD/DO degree, I am proud to be a DPM, and yes we do have an identity crisis. I do believe that some of this identity crisis is brought on by ourselves. I had a hospital board member who is also my top referring doctor ask me how many years of school we do. I had an ER doctor friend ask if it was okay to call podiatry for Lisfrancs or calcaneal fractures.

MDs/DOs and the general public do not know what we really do. We should be spending the time and money on educating people about who we really are, the advancements in our education and training. If we keep on referring out the rearfoot/midfoot traumas or deformities to foot and ankle orthos instead of our fellow DPM's, what does that tell the MDs about us?

I can tell you what a local foot and ankle MD calls our DPMs who refer cases to him,...

Editor's note: Dr. Lam's extended-length letter can be read here.
