Bunion Surgery Has Evolved: PA Podiatrist
Kevin Naugle, DPM, residency director at the Reading Hospital for foot and ankle reconstructive surgery, said bunion surgery has significantly evolved over the years. Years ago, patients having bunion surgery had to be hospitalized for days, the recovery period was painful, and after a time, it was possible the bunion could re-appear, Dr. Naugle said. That's because the best practice at the time was simply to remove the bump without correcting the underlying cause. “It's the angular deformity that needs to be corrected,” Naugle said.

Today, before surgery is scheduled, Naugle discusses with patients the surgery that's right for them. He determines if the bunion is mild, moderate, or severe, how much arthritis is in the joint, and the location and size of the bunion. “Many factors will dictate which procedure we do,” Naugle said. Many times, tiny plates and screws will be used to correct the angle of the bone. It's not just cutting the bump: It's an alignment change and that means it will need something to hold it in place,” Naugle said. “You're reconstructing the deformity to a normal state.”

MI Podiatrists Appointed to Board of Podiatric Medicine and Surgery
Governor Gretchen Whitmer appointed several people to the Board of Podiatric Medicine and Surgery August 9. Erik Kissel, DPM, of Bloomfield Township, is a podiatrist with Foot and Ankle Specialists of Southeast Michigan. He earned his Doctor of Podiatric Medicine from the Rosalind Franklin University School of Medicine and Science. Dr. Kissel is appointed to succeed Kevan Kreitman, DPM whose term expired June 30, 2019, to represent podiatrists for a term expiring June 30, 2023.

Over 100 Residents Complete in 16th Academic Rumble
The PRESENT Podiatric Residency Education Summit East 2019 took place at the Teaneck Marriott at Glenpointe NJ this weekend. The well attended 16th Residency Rumble was won this year by a very impressive combined team from Montefiore Medical Center in the Bronx, NY and Good Samaritan Hospital Medical Center in West Islip, NY. Vincent Gramuglia, DPM, Director, Pooja Shah, DPM, and Alisha Poonja, DPM competed for Montefiore; Amanda Khoury, DPM, Maria Cifone, DPM, Michael Mazer, DPM, and Kevin Douglas, DPM competed for Good Samaritan.

CA Podiatrist Delivers Andrew Clarke Honorary Keynote in South Africa
Dr. David G. Armstrong, Professor of Surgery and Director of the Southwestern Academic Limb Salvage Alliance (SALSA) at Keck School of Medicine of University of Southern California (USC) delivered the Andrew Clarke Honorary Keynote at the 12th Biennial Podiatry Association of South Africa Congress held in Cape Town this week.

The lecture is named for Andrew Clarke, who is credited with founding modern podiatric medical education for the nation. Armstrong joined colleagues from across the continent and worldwide. Armstrong also provided two other keynote lectures describing the past, present, and future of limb preservation.

6 Ways to Improve Your Revenue Cycle Management - Part 5
Although most practices have little control over patient coverage types, they can better mitigate financial risk by understanding all the components that impact revenue. Here are six ways to improve your practice’s RCM reimbursement tactics.
5. Monitor payment trends - It is the practice’s responsibility to monitor changes to payers’ guidelines and fees. Luckily, the Internet is a great resource, and pertinent updates can easily be found online by visiting the appropriate billing-related websites. Proposed changes to Medicare’s fee schedule, for example, are available through its website and the online Federal Register. The public typically has an opportunity to comment on proposed changes, which can help influence the final rule.

Query: Denial of CPT Code 20550
We are having denials for payment for CPT code 20550 with M72.2 and pain codes, as well. This is happening with Medicare and a couple of other payers. Any thoughts on this issue?

Response: Your MAC will not pay for a “dry needling” injection. If your chart supports the injection of a steroid, your claim should include a HCPCS code for a steroid (e.g. J0702). Also, you will need to include modifiers and narrative information reflective of the NDC number of the medication (each brand and generic has its own) and dosage.
Unfortunately, your MAC does not seem to have an LCD covering this issue. You may wish to call your carrier to see if this is the case. If so, you will likely need the assistance of your EHR/EMR vendor on how to place this information on the claim.

Document that you have ruled out by lab studies, or have sent this patient for evaluation by his primary care physicians, all lab work-ups which may suggest secondary gout from another kidney disorder. Though this presentation may appear as a form of primary "gout" (gouty attack), document any cardiovascular disorder, which may present with unilateral foot/ankle swelling as well.

This, of course, is a personal decision. My perspective is that my main job is taking patients who are symptomatic, and making them asymptomatic. If patients are pain-free, then they are already at my target goal.
The other complex aspect of these decisions is trying to project disease course. I really don't feel that any physician can accurately predict if and when a bunion will become symptomatic. How many times have we all seen an elderly patient come in with asymptomatic bunions? If we had an accurate crystal ball, then the decision to operate on asymptomatic deformities would be clearer.
My vote would be to leave it alone, and periodic visits to assess for symptoms or developing arthritis or other concerning criteria that would suggest the need for intervention.

Football Player Suffers Frostbite from Whole Body Cryotherapy
Oakland Raiders wide receiver Antonio Brown reported to training camp with the Raiders and team doctors discovered he was suffering from serious frostbite on his feet. Over time, this has resulted in the skin sloughing off his feet in sheets, and that has led to his being able to participate very minimally in training camp thus far.

Many athletes are now opting for Whole Body Cryotherapy (WBC) as a means to either supplement traditional ice baths, or replace them entirely. This involves standing in a sealed room and being exposed to extremely cold, dry air for a period of 2-3 minutes. Temperatures are normally below -100 degrees, with liquid nitrogen and refrigerated air being used to blast the person receiving treatment. According to a report from Pro Football Talk, Brown entered the cryochamber without appropriate footwear. This caused the frostbite, which is now a serious concern for the Raiders as the regular season draws near.
