NY Podiatrist Discusses Pros and Cons of Calluses
Calluses may be unsightly or even annoying, but that doesn’t mean they’re always bad for your health since they offer protection, according to podiatrist Johanna Youner, DPM of Park Avenue Podiatry Center in New York. For example, in a healthy person, calluses, especially in the summer when wearing sandals, can help prevent blisters from forming, said Dr. Youner.
Dr. Johanna Youner
There are, however, instances when calluses can have serious health consequences. Dr. Youner warns, if someone suffers from diabetes, calluses can prevent them from feeling pain and can, thus, lead to health issues. “If you feel pain,” she warns, it is time to see a doctor.

PODIATRISTS AND SPORTS MEDICINE
CT Podiatrist Stresses Importance of Hydration During Summer Workouts
According to podiatrist Robert Weiss, DPM, "When participating in any summertime sport, it is extremely important to maintain proper fluid levels. It's not the heat of the summer that is the enemy, but rather the inability to acclimate our bodies to the heat and hot sun. Some research indicates that you should drink 50 percent more than it takes to satisfy your thirst.
Dr. Robert Weiss
"Since most of our body's weight is water, it's still one of the best fluid replacements. If you don't drink enough, your heart and the rest of your body will not get enough blood and oxygen. At least one hour prior to your exercise workout, you should drink 8-10 ounces of water and continue to drink the same if you plan to exercise for more than an hour. Then continue to drink every 20 to 30 minutes with ongoing exercise," says Dr. Weiss.

CMS Expands Medicare Coverage of Blood Pressure Monitoring Device
The CMS has announced that it has extended coverage of blood pressure monitoring devices to all Medicare beneficiaries suspected of reporting abnormal blood pressure levels when administered in clinical settings. The device, called ambulatory blood pressure monitoring, is helpful for clinicians with patients who are nervous in clinical settings, affecting the accuracy of their blood pressure results. The devices track patients' blood pressure throughout the day and the accrued data allows doctors to understand patients' blood pressure during normal activity.
The agency previously only covered the use of the device, which monitors blood pressure periodically over a 24-hour period, for patients with suspected elevated blood pressure levels due to anxiety from being in a clinical setting. The device can now also be used for patients suspected of having lower than usual blood pressure measurements when inside a doctor's office. Medicare will cover the use of the device once a year per patient.

Appellate Court has ACA's Individual Mandate in Crosshairs
A federal appeals court may be poised to deem the ACA’s individual mandate unconstitutional, but appeared to be on the fence about whether they believe that strikes down the entire law. States on both sides of the constitutional debate faced off before a three-judge panel in the Fifth Circuit court Tuesday afternoon, with the future of the Affordable Care Act hanging in the balance. The judges seemed to wrestle with the question at the crux of the ongoing case—can the individual mandate be excised from the law?
Texas is leading a group of 18 red states that argue Congress' 2017 decision to set the mandate penalty to $0 prevents it from functioning as a tax. The Supreme Court narrowly ruled in 2012 that the individual mandate for everyone to get insurance could be enforced as a tax under the Interstate Commerce Clause. The states argue that since that tax has been effectively struck down, then the rest of the law must go as Congress lost its tax authority.

FROM PM's CURRENT ISSUE
Several times a week, PM News posts an entire article from a recent issue of our partner, Podiatry Management Magazine. Please note that the views expressed in Podiatry Management Magazine do not necessarily reflect the views of PM News or Barry Block.
Reputation management software tools are easy to implement. Besides making it much easier to ask patients for new reviews, most offer other bells and whistles (e.g., text reminders), and often give you updates when new reviews are posted on key sites.

Query: Affordable Amniotic Membrane Product
I have been using AmnioFix injectable for plantar fasciitis and Achilles tendinitis. Recently, they have begun the process of getting FDA approval for these and similar conditions and were told they had to remove it from the market and only allow the sheet forms to be available in the OR. This raises the cost dramatically. Is anyone using any other amniotic membrane product with good results? One that remains around $225 or so?

RE: Failure to Diagnose PAD
From: Allen Jacobs, DPM
Increasingly, I have observed that plaintiff attorneys specializing in litigation involving care of the elderly are looking at podiatry care. One major area is the failure to diagnose and treat or refer for treatment geriatric patients with PAD.
The argument being advanced is as follows; the patient is qualified for care by a podiatrist by virtue of class findings consistent with PAD. Therefore, why was the patient not referred for vascular studies or to vascular surgery or interventional cardiology? Given advanced age and the common presence in elderly patients of comorbidities with which PAD is associated, a potentially compelling argument is that

I am interested in the new Podiatry Guild insurance for podiatrists and their families. How do I join the Guild? Is this available to retirees also?
Ilene S. Terrell, DPM, Fredericksburg, VA
Response: This month, the Office and Professional Employees Internal Union (OPEIU) has provided a Health and Welfare Plan that is open to each OPEIU Member and their families. Local 45 of the OPEIU (The Guild) has made this plan available to its members. Membership in Local 45 (Guild) comes through a DPM's membership in the state podiatric medical association which has affiliated with the Guild.
The OPEIU Health and Welfare Plan offers three (3) fully insured insurance plans through a Blue Cross Blue Shield "Blue Card" that allows members and their families, to...

Congratulations to Barry Block on being appointed to the editorial board of CPQ Orthopaedics. He has established something from nothing, meaning that he has created the most widely read daily news source for podiatrists in this country and worldwide. I don't know if all PM News readers appreciate the amount of effort and work on a daily basis involved with this. The dedication he has to the Podiatry Management magazine and PM News publications is apparent. Most of all, congratulations to Dr. Block for developing and continuing to run the most widely read open access podiatric daily news source.
