OH Podiatrist Discusses Why Your Toenails Might Turn White
"The human nail is a complex structure generally made up of a collagen complex called keratin (the same cells that make up your hair). The visible portion of the nail plate that people see is actually comprised of dead cells, and that is a reason why they don’t typically hurt when you cut them," says Mark J. Mendeszoon, DPM, a podiatrist at Precision Orthopaedic Specialties in Ohio. "Healthy fingernails typically grow about 3.5 millimeters per month and toenails grow 1.5 millimeters per month."

So if you've noticed white spots or marks on your toenails the last time you were clipping your toenails, you might be a little concerned. "At times, people may notice white discoloration on their nails, and this can become frustrating, concerning, or even embarrassing to individuals," says Dr. Mendeszoon. One of the most common causes of white toenails or spots is a fungal infection (onychomycosis) or a yeast infection (Candida). "Autoimmune conditions like psoriasis may cause white spots or pits in the nails," says Dr. Mendeszoon. "In addition to white discolorations of nails, iron deficiency could cause growth disturbances of the nail, such as a spoon-shaped nail (koilonychia)."

MO Podiatrist Reviews Comprehensive Management of the Diabetic Foot
Allen Jacobs, DPM was recently interviewed on Ben Pearl, DPM's podcast, Fit Foot U. Dr. Jacobs expressed his concern about the neglect of medical aspects in the podiatry profession, particularly in the area of diabetes care. Dr. Jacobs stated that the focus has been primarily on surgical procedures and the prevention of diabetic ulcers, rather than addressing the multifactorial issues that impact the quality of life of diabetic patients. He emphasized the need for a more comprehensive approach to diabetes care, including the treatment of neuropathy, evaluation and treatment of vascular disease, and fall prevention, among others.

Dr. Jacobs noted that basic clinical testing is 87% accurate in detecting neuropathy and that for the most part epidermal nerve fiber testing is unnecessary. He concluded by stating that he sees a bright future for podiatry within multispecialty groups. He is eager to hear new voices within the profession, rather than the same old speakers on the podium.

RE: Has APMA Appointed Future Action Strategists?

At all times, healthcare professional organizations responsible for public outreach and relations usually have appointed future actions strategists. These qualified persons constantly monitor the standing its members have in changing aspects within the healthcare marketplace, including any significant changes in reimbursement issues.
I have been a member of APMA since 1958, and I am hoping that the APMA has done now what it has done in the past by appointing qualified strategists during these rapidly changing times (such as was done prior to change of the name of the profession from chiropody to podiatry). If it has, these strategists are realizing that the quickly advancing Medicare spearheaded transition from fee-for-service reimbursement to value-based care (VBC) is already devaluing payment for elective, "non-life threatening” surgery, and it is increasing the reimbursement value of prevention and chronic disease management E/M services. In podiatric medicine, chronic diseases include, but are not limited to diabetes peripheral neuropathy (DPN) and peripheral artery disease (PAD).
I am concerned about this because recent board certification discussions that appear to ultimately put, "most of all of podiatry's eggs in one surgery basket" could, to say the least, be counterproductive for the profession of podiatry.

RE: Observations on the Changing Face of Medicine

I have a patient who is a soon-to-be retired psychiatrist. Whenever he would come into the office, we would have discussions on many topics not related to psychiatry or podiatry. He was very well aware of all facets of general medicine. If I asked this physician a medical question, he knew the answer. He later told me that he works one day a week in the ER doing emergency medicine and this helps him keep up with the entire medical field. In another case, my former GI specialist who just retired was able to comment with interest and expertise on any medical issue aside from the GI system.
I am now faced with seeing a whole new battery of young specialists in different fields and if I ask them a question outside their specialty, their answer tends to be, "It's not my field, go to an appropriate specialist." I am seeing more and more of this happening and some of these doctors are board certified in internal medicine and if the question does not directly relate to their subspecialty, the wall in the room can give me a better answer.
As a podiatrist, this may be good. We are specialized and only responsible for the foot, and so many other young specialists seem to have developed amnesia to all aspects of medicine other than their own narrow specialties. Perhaps the degree given to these physicians should not be an MD or DO but for example, "doctor of orthopedics or doctor of oncology, etc. Being a doctor of medicine is becoming less and less relevant in today’s practice.

The fact of the matter is, a DO does additional training above that of the MD, yet the AMA tries to minimize their degree and strong arm them out of competition. They instill this thinking into their graduates. I remember touring a potential MD school I was considering attending. We were given a tour by an MD resident. I asked if she ever considered DO school. Her reply was, “I didn’t consider it. After all this training, I want to be respected.” I tried to keep from laughing.
We are on a path of stepping stones as DPMs, with the hopeful outcome of obtaining an MD or DO degree in addition to DPM. It may not sit well with some, but the reality is, medicine continues to change. The DO degree only has to deal with disrespect. DPMs have to deal with disrespect, lack of full scope licensure, and a small political presence.

I agree that removing the offensive post is not enough; a retraction with appropriate language is what is needed. The damage is done without the retraction quote. Otherwise, SUE. I would ask nicely for a retraction and if they do not respond, threaten a suit. The AMA are not our friends, as shown with this physician’s issue. The AMA needs to be dealt with appropriately until they give us the correct due respect. Just like in war, use deterrence with strength; cowering to them gets us nowhere.

As a follow-up to the discussion regarding the AMA's denigration of podiatry, it doesn't stop there. As most readers of PM News know, I have been practicing functional medicine for 37 years. I was among the first doctors in the country to study under Dr. Jeffrey Bland, the "father" of functional medicine in this country.
The Institute for Functional Medicine offered no certification to podiatrists for many years. They do now. But, we are offered certification as "non-medical" professionals. The medical certification is offered only for the following degrees - MD, DO, DC, ND (naturopathic doctors), NPs, and PAs. We do not qualify for a medical certification but chiropractors do? Physician assistants do? Podiatrists are offered certification under "Certified Professionals", which puts us with acupuncturists, optometrists, pharmacists, physical therapists, psychologists, social workers, nutritionists, occupational therapists, speech pathologists, dentists, and RNs.
I blame this on our national and state organizations who clearly have done nothing to improve our image and communicate to society exactly what we are trained and licensed to do. We can complete a 3-year residency, prescribe any drug or narcotic, perform all kinds of foot and ankle surgery in a hospital OR and yet, we are not "medical" professionals? Insane. As a 1980 graduate of NYCPM, I would have thought after all of these years that this would no longer be an issue for us.
