APMA NEWS
Victory! AMA Removes Offensive Social Media Post

APMA has announced that AMA voluntarily removed the offensive and inaccurate social media post and the corresponding article on its website attacking podiatric education and training. This decision by AMA is a direct result of APMA's multipronged efforts on its members’ behalf. The AMA post, which appeared on AMA's X and Facebook accounts on June 16, noted that "many patients wrongly believe podiatrists are physicians" and linked to an article riddled with half-truths and inaccuracies about podiatric physicians' education, training, and experience.

APMA's Board of Trustees is communicating directly with AMA leadership and hopes to arrange a meeting with AMA volunteer leadership in the near future to continue the conversation around this issue. APMA will continue to vigorously defend member podiatric physicians' and surgeons' ability to practice to the full extent of their education and training to best serve the public health.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Biggest Barriers When Moving to Value-Based Care

The U.S. Centers for Medicare & Medicaid Services (CMS) continue to make health equity a key aspect of value-based care. Studies are increasingly detailing how the conditions in which people are born, grow, live, work, and age – what are known as the social determinants of health (SDOH) – have an enormous impact on health outcomes.

5. Choosing Effective Tooling

Here are some questions for ACOs to consider when choosing a plan:
  • Is the tooling built on considerations of SDOH, or is SDOH just an add on at the end? Tooling that only considers SDOH at the end will likely waste money since it assumes a standardized recommendation and only triages for SDOH after pre-selecting options.
  • Can the tooling determine SDOH for specific patients? The technology needs to go beyond ZIP codes, more specific than communities, to give a holistic picture for particular patients.
  • Does the tooling adapt with changing data? For tooling to be effective in practice, it should continually update and adjust with new patient information so it can grow with an ACO as new patients join.
Bringing effective tech into ACOs to drive measurable, data-driven health equity results will cost money. ACOs can save by using tooling that is proven and already built, rather than designing their own.

QUERIES (NON-CLINICAL)
Query: Financing Companies for Podiatric Services

For many years, we have been utilizing CareCredit from Synchrony Bank to finance podiatric services for patients who need/want to have these services performed,  but could not afford to pay for them in full. Recently, we have noticed a significant decline in their level of customer service and we have decided to close our account with them.

I am hopeful that some of my colleagues who have had positive experience with other companies offering similar financing services can please share their experience and the name and contact information for that company.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
RE: Podiatrists Self-Identifying as" Dr." or "Physician"
From: Carl Solomon DPM

Putting aside the discussion of whether or not podiatrists are physicians, I’d like to express some thoughts about how we address ourselves in that context. I can hardly recall an instance in which one of my MD/DO friends addressed him/herself as “physician”. When asked, or introduced, it’s “I’m an oncologist, orthopedist, rheumatologist, general surgeon…”, whatever. My dentist friends aren't too proud to be identified as a dentist...perhaps when appropriate, oral surgeon. Not physician.

And when I see the use of the term “Dr.” written in front of somebody’s name without other explanation, almost without exception, that’s a de facto acknowledgement that “I am not an MD”. This was personified  on the sign-in sheet at one of my hospitals’ Dept. of surgery meetings. Everyone signed in simply with their name, with the exception of two, each of whom signed “Dr. Xxxxx”. You can guess…

A podiatrist may achieve some level of recognition by assigning him/herself the title of "Dr." or "physician", and the inference is that it represents being an MD. But oftentimes once the details come out, the concealment of the identity as a podiatrist makes it apparent that there is a low level of self-esteem associated with such identity.  So whatever special recognition may have been achieved by initially identifying as a Dr. or physician, is actually negated. On the other hand, if one is humble and is acknowledged by another party as being a doctor…podiatrist, the level of recognition will remain high. If you can’t take pride in being a podiatrist, you should have spent the time and effort doing something else!

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2
RE: A Rose by Any Other Name…
From: Rod Tomczak, DPM, MD, EdD

I hope all who read the letter by ACFAS and APMA have also looked at the history of the resolutions and position papers. It seems that in 2020 someone came up with the idea that if students of podiatric medicine could pass USMLE, they should be classified as physicians.

Out of the right side of their mouths, the MD officials are pushing our students to take the USMLE, and out of the left side, they are saying MD and DO passers of the USMLE can call themselves “physicians.” What seems very odd is that DOs can still take their COMLEX exam and be licensed as physicians.

This whole ploy is the biggest example of obfuscation I have ever seen. AOFAS would rather partner with Reiki practitioners than...

RESPONSES/COMMENTS (ARTIFICIAL INTELLIGENCE)
From: Robert Scott Steinberg, DPM

I did a Google search on "Where can I go to get this big, red, sore bunion taken care of?" I suspect 90% of people use Google search. The results were not good for podiatry.

RESPONSES/COMMENTS (PODIATRY AND SOCIAL MEDIA)
RE: APMA's Response to Egregious Messages from AMA (Elliot Udell and Lawrence Rubin, DPM
From: Paul Kesselman, DPM

Both Drs. Udell and Rubin provide a wonderful synopsis of the issues we face and will continue to face. Since these postings appeared, APMA has announced that AMA has withdrawn the posting. Those who wrote to AMA or spoke with their MD colleagues demanding it be retracted are to be applauded for doing what we do best...communicate what podiatrists contribute every day to the healthcare team.

Today, an eye-opener appeared on my desk. An insurance carrier asked for a review of 1,300 pages of medical records for a mid ‘50s well controlled diabetic who sustained an unfortunate blunt force crush injury. The highly trained MDs (vascular and ortho) at a well known tertiary care hospital he was admitted to wanted to amputate this patient's...
