The Special Olympics of Maryland held their annual competition at Towson University, in Towson, MD, on June 22.  Podiatry was one of the nine medical disciplines present.

5 Biggest Barriers When Moving to Value-Based Care
 
The U.S. Centers for Medicare & Medicaid Services (CMS) continue to make health equity a key aspect of value-based care. Studies are increasingly detailing how the conditions in which people are born, grow, live, work, and age – what are known as the social determinants of health (SDOH) – have an enormous impact on health outcomes.
 
4. SDOH act as co-morbidities: Treat them as such - Most importantly, ACOs need to take seriously the idea that SDOH act as a comorbidity. In a value-based care model, ACOs’ bottom line depends on it, as does the health outcomes of their patients.
 
Without a data-driven approach to how to address SDOH on a patient-by-patient basis, ACOs are almost guaranteed to lose money while not actually improving the quality of health outcomes. And providers will suffer as a casualty if effective methods are not employed.

In July last year, I penned a letter to the editor of Podiatry Management voicing my disappointment regarding the concurrent in-person and virtual attendance options at conferences. To their credit, The California Podiatric Medical Association made the bold choice to exclusively host in-person attendance at The 2024 Western Foot and Ankle Conference. As a result, foot traffic in the exhibit hall notably surged compared to the previous year. I extend my gratitude to all parties involved in making this decision.

In response to the concerns of Lawrence Rubin, DPM that “present and future bad AI public information” is harming us, as a test to see if these concerns are well founded, I prompted the 4 AIs that I use regularly with this sample patient question: "Where can I go to get this big, red, sore bunion taken care of?". The responses were interesting and generally constituted good advice. I am happy to report that Perplexity, ChatGPT, Claude and Gemini suggest seeing a podiatrist FIRST. The first 3 mentioned orthopedists as a SECOND choice. The last didn't mention orthopedists at all. What this exercise tells me is that our institutions that are charged by us with getting the information as to podiatry’s superiority for treating the foot and ankle out to the public, at this time, have successfully done that.
 
We are often inclined to criticize the APMA, ACPM, ACFAS when we see stats on the low public awareness of what a podiatrist is. Well, this is strong evidence that they are actually doing a good job with...

It is saddening to once again see some literature sanctioned by the American Medical Association which "smells" of prejudice. Having been in practice for over 40 years I still remember the not-good old days when podiatric surgeons were only allowed to do surgery in their offices, no matter how complicated the cases were. In my geographic area, we were lucky. There was an osteopathic hospital that allowed podiatrists to do surgery. At that time, osteopathic physicians were also at the receiving end of bias and prejudice. Misery likes company. Hence, Massapequa General Hospital was the only home for DOs and it welcomed podiatrists. 
 
Today, graduates of DO schools are treated on the same level as MDs and podiatrists are on the surgical staff of almost every hospital. This does not mean that for one minute we let our guards down. The ugly head of bias and prejudice can also come up.  Prejudice against DPMs is no different than any other form of bigotry and we need to be vigilant to prevent a return to the past.

In Dr. Kesselman's usual analytic manner, he posited that the intent of the AMA message may have been to save themselves and look good to AAOS members. I believe there may be another reason. Medicare Advantage and other large self-funded insurance entities are rapidly moving forward to the Value-Based Care (VBC) capitated model in which foot surgery and providers of foot surgery become business commodities. 
 
ACOs will have to make individual decisions about their choice of capitated foot surgeons. If you were the financial director of an ACO, would you hire a controversial provider who some believe is not even a physician when your present orthopedic surgeon is capable of performing foot surgery? I believe strong efforts should be made to be able to prove to healthcare purchasers that podiatric surgeons have better patient outcomes, and this leads to optimal value-based care.

I wholeheartedly agree with Dr. Ribotsky about forming a cancer registry for podiatrists. I too, am a cancer survivor, finding out I have lung cancer following my retirement after 41 years of practice. I am a lifelong non-smoker and can only guess what the source of my cancer is, but I think of all the carcinogens I was exposed to during my working days. My advice to younger practitioners is to protect yourself from radiation exposure and assure the air quality in your environment is as free of carcinogens as possible. Check your homes and offices for radon, considered the leading cause of lung cancer in non-smokers, and also consider having a low dose CT screening scan of your lungs if over 50 years old.

Reflecting on my four-decade-long journey in podiatry, I am reminded of the persistent misconception that we, as podiatrists, should strive for 'parity' with MDs, as if we are somehow inferior. This misguided notion often arises from comparisons with our orthopedic colleagues and their inherent positions of power, as recently evidenced in The AMA’s pathetic comparison of orthopedics with podiatry. This misinformation often overlooks the unique aspects and strengths of podiatry as its own distinct medical and surgical specialty. Here are several points in defense of specialized certifications within podiatry under a single board while also emphasizing the need for us to set our own standards:

I was shocked to hear of Glenn's passing. He was a friend and mentor for over 30 years. Glenn gave me my first job in podiatry and taught me so much about the business of podiatry. He was patient, kind, generous, and a great teacher with an incredible work ethic. In the years that passed, I would stop by his office every other year or so just to say hello and catch up, and these always turned into hour long conversations about life.
 
My sincere condolences to his children, Ben and Kayla. Glenn was a special man who genuinely loved people, his beloved pets, and skiing. He will be missed by all. Rest in peace, Glenn.
