Podiatrist Discusses How to Deal with Runner's Subungual Hematoma
 
James Yakel, DPM, a sports medicine podiatrist based in Colorado and a runner himself, says that the actual term for runner’s toe is subungual hematoma. “What happens is, the longest toe—which can be either the big toe or the second toe—is experiencing repeated trauma by jamming into the shoe, which causes bleeding underneath the nail,” he says.

Dr. Yakel says you have between one and five days to drain the blood before it dries. To do this, you can book an appointment with a podiatrist who is trained to do this kind of thing, or if you’re not squeamish, you can attempt to do it yourself. Again, you don’t have to do this. If your black toenail isn’t causing you any pain, you can leave it alone.

PROFESSIONAL MISCONDUCT
'Microaggressions' Could Cost Doctors Their Licenses in OR Under Proposed Rule
 
The Oregon Medical Board has introduced a new ethics rule that threatens to revoke the medical licenses of doctors who commit "microaggressions" in the workplace. Under the proposed law first introduced in April, doctors will be required to report "unprofessional or dishonorable conduct" by themselves or a licensed colleague within 10 business days. If they do not comply with the mandatory rule, they will face disciplinary action, which could include the loss of their medical license, according to the proposal.
 
Under the rule, the term "unprofessional conduct" would be expanded to include "discrimination in the practice of medicine, podiatry, and acupuncture" through "unfair treatment characterized by implicit and explicit bias, including microaggressions, or indirect or subtle behaviors that reflect negative attitudes or beliefs about a non-majority group."

5 Biggest Barriers When Moving to Value-Based Care
 
The U.S. Centers for Medicare & Medicaid Services (CMS) continue to make health equity a key aspect of value-based care. Studies are increasingly detailing how the conditions in which people are born, grow, live, work, and age – what are known as the social determinants of health (SDOH) – have an enormous impact on health outcomes.
 
3. Data-driven approaches work- SDOH is a complex, data-driven discipline, and CMS is approaching it as such. The equity plan requirement is not simply about cobbling together some high-sounding sentences. Rather, it demands a data-driven workflow tool for assessing SDOH for particular patients, tech that can be monitored on an ongoing basis. Such tooling needs to provide actual data for tracking and monitoring outcomes. Even achieving minimal compliance will require a sophisticated and technical approach, one based on data science and engineering.

I am confident that AMA will receive numerous responses from organizations and individuals regarding the recent statement criticizing the qualifications of podiatrists and our role in the healthcare team. Thus, I will not address all the misleading or inaccurate points in AMA's statement; instead, I simply urge AMA to be more professional and collegial in your future communications. The substance of AMA's statement is unbecoming of the AMA, a respected institution. Undoubtedly, these offensives threaten the collaborative relationship between the AMA and the APMA, in which we stand united on many issues for the benefit of patients and providers.
 
It is illogical to criticize podiatrists for not being MDs with a full scope of practice. Every specialist, including podiatrists and orthopedic surgeons, works with other medical professionals in a team approach to ensure that patients receive...

I have been practicing podiatric medicine and surgery for almost 26 years. During my career, I have seen various organizations and news entities discussing podiatrists and comparing us to allopathic practitioners. In my experience, these entities do this because they feel podiatrists are a threat to allopathic colleagues. As we all know, we see many second opinions from patients who have been misdiagnosed or have had poor surgical results by orthopedists.  It is only fair to say that the orthopedist has had to treat our patients similarly. It goes both ways. It will always be like this. It is just the nature of having two professions treat the same body part. 

This situation occurs with plastic surgeons, oral maxillofacial facial surgeons, and ENT physicians. It is also seen with dermatologists and plastic surgeons, general surgeons, and highly specialized subspecialties within surgery. As podiatric physicians and surgeons, our primary focus is on diagnosing and managing our patients, particularly in the lower extremity. We lead by example, demonstrating our knowledge and dedication to providing the best possible care. The public has witnessed our capabilities, and it's crucial that we continue to excel in healing our patients, always putting their needs first. 

I explain to my patients that it is in their best interest to have a practitioner who sees nothing but the lower extremity manage their condition. I explain to them that our training is comparable to that of allopathic physicians, and our concentration is on the lower extremity. Yes, we didn't have training in gynecology, obstetrics, psychiatry, advanced cardiology, or managing rotator cuff injuries, but we have had much more training in biomechanics, lower extremity wound, care, etc. Let’s expect that we will always have other organizations feel threatened by us only because we will continue to excel in healing our patients.

RE: AI and Podiatry

As podiatry board certification matters are bringing the profession closer and closer to there being mainly surgery limited practices, we'd better figure out a way to combat present and future bad AI public information. Let's say a footsore person gives AI this question: "Where can I go to get this big, red, sore bunion taken care of?" 
 
It surely will not be to the benefit of the patient or podiatric surgeon for the AI answer to be, "Go to the Walgreens store nearest you. Ask the pharmacist where you can find bunion shields to reduce pain and inflammation caused by shoes and also Tylenol for bouts of intense pain." Forewarned Is Forearmed. We have to prepare ourselves to deal with AI. It’s good to see that APMA will have a lecture on AI at its August meeting.

Kudos to Dr. Ribotsky for suggesting that there be some way of tracking podiatrists diagnosed with some form of cancer. As a cancer patient in remission, this issue is very close to me. 
 
The medical community is grappling with another issue. Patients with breast and colon cancer are now presenting at very young ages. One patient of mine had his first colonoscopy at age forty and discovered that he had stage four colon cancer. Another young woman in her thirties is undergoing treatment for breast cancer. Are these caused by unidentified carcinogens or are people discovering these conditions earlier in life because of testing and awareness?

Dr. Ribotsky brings up an excellent point about developing a cancer registry within our profession. What becomes very concerning to me is the point he brings up about the dangers of podiatrists and their assistants breathing in nail dust, a known carcinogen. With all the OSHA regulations medical practices must adhere to, regulations regarding inhalation of dangerous nail dust does not seem to be a concern of OSHA. 

I've been manufacturing and selling a nail dust extractor system for many years and I cannot begin to tell you the frustration I have when I see the infinite regulations physicians must adhere to while the nail dust produced by one of the most common procedures podiatrists do is not addressed. I have sold many vacuum units throughout the years but would conservatively estimate that 80-90% of podiatry practices doing routine podiatry do not incorporate nail dust extraction systems. 
 
Dr. Ribotsky brings up an excellent idea about forming a cancer registry. Unfortunately, it may be an uphill battle when governmental agencies such as OSHA make demands on offices that are borderline ridiculous, while known airborne carcinogens such as nail dust go completely unregulated.

This week baseball legend Willie Mays passed away at the age of ninety-three. My father Merton Root, DPM had several of the San Francisco Giants as patients. Gaylord Perry gave my father tickets to a game for my birthday and invited me to come into the clubhouse before the game to meet the players. I idolized several of the Giant's players, especially Willie Mays.

While walking around the locker room being introduced to players by Gaylord Perry, Willie Mays invited me over to his bench for a picture with him. I think I was speechless. I will never forget how kind and warm he was. What an amazing athlete, role model and human being he was.
