AT THE COLLEGES - PART 1
Manhattan Borough President Visits NYCPM Touro

Manhattan Borough President Brad Hoylman-Sigal recently toured the Foot Clinic of New York (FCNY) at the New York College of Podiatric Medicine at Touro University (NYCPM), gaining a firsthand look at the clinic's essential role in delivering foot and ankle care to some of Manhattan's most medically underserved communities.

(L-R) Drs. Loretta Logan and Alan Kadish; Arthur Goldstein, Dr. Mike Trepal, and Brad Hollyhman-Sigal

The tour was led by Touro University President Alan Kadish, MD, and NYCPM Dean Michael Trepal, DPM, alongside other senior Touro and NYCPM leadership. The visit was part of the clinic's effort to secure a grant from the Manhattan Borough President's office to acquire needed clinical equipment – an investment that would expand the FCNY's capacity to serve patients while strengthening the training experience for the next generation of podiatric physicians.

PODIATRISTS IN THE NEWS
TX Podiatrist Raises Awareness About "Hidden Mobility Loss" in Adults Over 40

Hope Jacoby, DPM, owner of Texas Wound Pros and Foot and Ankle Pros, is raising awareness about what she calls "hidden mobility loss" -- the gradual decline in movement and stability caused by untreated foot and ankle issues. According to Dr. Jacoby, many adults over 40 adjust to pain slowly without realizing how much their daily movement has changed.

Dr. Hope Jacoby

"People don't usually wake up one day unable to move comfortably," says Dr. Jacoby. "It often happens gradually. They stop walking as much. They avoid stairs. They limit activity without fully noticing it." Foot pain, chronic stiffness, ankle instability, and untreated injuries can slowly affect posture, balance, and physical confidence. Over time, these small adjustments may contribute to larger mobility problems.

PRACTICE MANAGEMENT TIP OF THE DAY
8 Steps to Build an AI Governance Plan for Your Practice

Here are 8 practical steps to govern AI in your practice, from naming an owner to vetting vendors and training your team.

8Train your team, not just your software

The best tool, underused or misused, is a liability rather than an asset. Invest in training so staff understand what a tool can and cannot do, where it tends to err, and how to escalate a problem. Most physicians say they want a direct hand in how AI is adopted, so build readiness around people, not just the software contract. The American Medical Association advises practices to decide who is accountable and build the rest from there.

FINANCIAL TIP OF THE DAY
7 Money Behaviors That Can Hold Back Financial Success

These everyday decisions that may be costing you more than you realize.

1. Don’t react impulsively with your money

Flash sales, "only three left," and market headlines often create urgency that feels smart in the moment and expensive later. Research finds that time pressure pushes people toward impulse buying. This is because impulse purchases are often about short-term emotional relief rather than long-term value. To protect yourself from impulsive spending, build a default pause into your purchasing process. For everyday wants, give yourself 24 hours before buying; for big-ticket items, wait 72 hours.

If you invest, keep a one-page plan that spells out why you buy, rebalance, or sell. If a decision doesn't fit your plan, don’t make it. Taking the time to pause and reflect will reduce regret and help you stick to your strategy when the temptation arises to spend based on emotions.

SOLE PURPOSE BY SAVANNAH SANTIAGO, DPM
Protect Your Podiatric Passion

Here are some valuable tips that have aided Savannah Santiago, DPM in navigating her experiences as a student and a resident:

Here's how to build a career in healthcare that is not only long, but meaningful, and how to do so without slowly becoming jaded along the way.

1. Have Hobbies

Residency has a way of becoming all-consuming, where your schedule, your energy, and often your identity revolves around the hospital. That reality is hard to escape, and in many ways, it’s part of the process. But within that intensity, it becomes even more important to hold onto the parts of your life that exist outside of medicine, the things that remind you who you are beyond your role as a resident.

These pieces of your life may seem small compared to the demands of residency, but they are not insignificant. They are what keep you feeling like a whole person. And in a field that asks so much of you, holding onto that sense of self is not just important, it’s necessary.

CODING & BILLING Q&As FROM CODINGHELPLINE.COM
Query:  Is it OK that my EMR Automatically Assigns MDM to Diagnoses?

When I select a diagnosis that may have been a chronically progressive problem, I look at the assigned value in my EMR and it says minimal. This potentially devalues my E/M to a level 2. The documentation shows it is at minimum a low risk but depending on my treatment could be a moderate risk. Why would the EMR assign the incorrect risk?

Codinghelpline com Subscriber

Response from Codinghelpline.comThe EMR has programmers who sometimes do not see or understand the variability in the diagnosis you selected. By defaulting it to the lowest level, it is your responsibility to update the diagnosis variability to what was documented. If it is a chronically progressing condition, that might end up being an E/M level 3 or 4. Toggle the diagnosis from minimal to the higher level based on your documentation and treatment provided, it most likely will not equal a level 2 E/M.

QUERIES (DIABETIC FOOT EXAMS)

Query: Diabetic Foot Exam as a HEDIS Measure?

I started inquiring about getting the diabetic foot exam approved as an official HEDIS Measure about 10 years ago to both the ADA, APMA, and the NCQA. My multiple attempts at getting any meaningful updates have been futile. Could anyone please provide an update on this very important topic?

RESPONSES/COMMENTS (INSURANCE COMPANY FRAUD)
RE: Reports on More Fraud by Contractors
From: Paul Kesselman, DPM

Two recent reports continue to illustrate the significant amount of fraud and abuse at the level of contractors, NOT physicians or suppliers. The first is the State of Massachusetts is suing UHC over alleged $100M in fraudulent Medicaid payments. Having read the summary of the complaint, it appears that for over the last decade, UHC has been involved in data mining and billing CMS for higher levels of care than were actually provided. The full story can be found here

The second is a report from the OIG entitled: “CMS Potentially Overpaid Medicare Advantage Organizations $462 Million Based on Certain Unsupported Acute Stroke Diagnosis Codes”. The OIG Found the following:

• For all 97 sampled enrollees,...
