UT Podiatrist Saves Foot of Woman with Charcot Foot
Debra Eaton of Clinton, Utah has diabetes and had not heard of Charcot or diabetic foot. Her foot started swelling, and she thought that she had been on her feet too much and felt that she needed to put them up after being on them all day. She had multiple fractures and was using a scooter for 18 months to keep weight off it. However, her foot did not get better, and Anna Hronek, DPM diagnosed her with Charcot foot, and told her that her foot was a bag of bones.

On October 5, 2022, she had surgery to reconstruct her foot, which included a 12-inch rod. She had to learn to re-walk. Eaton is grateful that Dr. Hronek saved her foot. Eaton went back to work in March 2023 but in a wheelchair. At the end of May, she finally got to wear shoes. She’s now looking forward to hiking. She wants others to be aware and to know their options.

MI Podiatrist Explains Heat Exhaustion Versus Heat Stroke
According to Conway McLean, DPM, "There is confusion by many regarding heat exhaustion versus heat stroke. The primary symptoms of the latter consist of a change in mental status, like confusion or delirium, and seizures or loss of consciousness may occur. When the water loss of sweating and the subsequent cooling is insufficient, core temperature rises."

"A temperature above 104 F is associated with heat stroke and is a medical emergency, with lowering core temp the only effective treatment (think ice baths). Heat exhaustion is different but can progress to heat stroke if untreated. It presents with dizziness, headache, nausea, weakness, and muscle cramps, as some common signs. With heat exhaustion, the pulse rate will be fast and weak, breathing fast and shallow," says Dr. McLean.

TX Podiatrist Provides Advice on Choosing Slides
Adding the most comfortable slides to your wardrobe makes getting ready a breeze. But just because the footwear requires minimal effort to, well, slide on, doesn’t mean it lacks support. However, “Slides are not recommended to be shoes worn for especially long periods of time because they often lack support and require extra work from your feet to hold slides on while you walk. They are best for leisure wear,” says Anne Sharkey, DPM, a podiatrist at the North Austin Foot & Ankle Institute.

Finding a shoe with a secure fitting vamp helps. “In order to keep slides on your feet, you have to grip your toes and lift your feet more with walking,” Dr. Sharkey explains. “Choosing slides that fit well and have adjustable straps can help reduce the work required by your feet to keep the slides on.”

Inaugural Chesapeake Bay Advanced Foot & Ankle Fellows Course
The Chesapeake Bay Advanced Foot & Ankle fellows course in Annapolis was held for the first time this weekend. Orthopedic and podiatric foot and ankle fellows and faculty were hosted from the U.S. and around the world including fellows from Korea, Türkiye, and Iran. The goal of this course was to bring together orthopedic and podiatric surgeons who simply want to learn and be better for their patients. Shruti Patel, DPM served as the organizer and course chair.

Morning lectures both days were followed by a lab session. Fellows and faculty practice procedures ranging from total ankle replacement, MIS, to high level trauma management and even complex Charcot reconstruction. Following the course, attendees took the water taxi across to dinner at Chart House where some of the faculty shared a few invaluable pearls for the fellows.

Aussie Podiatrist Warns of Dangers of Foot Peel Masks
Foot peel masks have become a viral sensation, promising baby-soft feet with minimal effort. But a podiatrist is sounding the alarm on the potential dangers lurking behind the popular beauty trend. Nicki Dang, known as @thepodlifts on social media, is cautioning the public about the health risks associated with foot peel masks. In a viral video, Nicki explains that these foot peel masks can actually cause serious harm.

"The mask doesn’t know the difference between healthy and dead skin," she warns. This inability to separate can lead to a range of health issues, from mild irritation to severe medical conditions. The most concerning risks are for individuals with underlying health conditions. Nicki said that people with diabetes or those who are ill need to be particularly vigilant.

RE: The Future of Podiatry (Alan Sherman, DPM)

Dr. Sherman is correct. Concierge medicine is a great business model for internists. One of my patients asked me for advice on this matter. His internist's practice was being flipped into a concierge practice and sought my advice on what to do. I went over the list of meds and conditions that this doctor was treating him for. It was extensive and he was happy with the way the doctor was managing all of his complex medical issues. My advice to him was to stay with the doctor if he could afford the 2k a year.
My own internist went into concierge. In my case, I only saw him for check-ups and most of my medical visits, at that time in my life, were to my allergist. Even today, most of my visits are to specialists. In my case, it did not pay and I easily found a substitute internist. To make matters worse, my medical insurance did not recognize concierge practices and I would have to pay cash for each visit in addition to the 2k annual fee.
The bottom line is that concierge medicine is working well for the internists involved but may or may not be workable for every patient.
