OH Podiatry Office Destroyed By Huge Fire
Fire swept through a historic 19th century commercial block in a quaint 150-year-old Portage County village Saturday. Thirty-four area fire departments responded to the call for mutual aid -- many of them bringing tankers of water to help fight the blaze -- but by late afternoon, Mayor Rick Patrick was not confident there was anything left to be saved. "It looks like we're going to lose that whole block," he said.

Some 13 businesses were affected, including a barber shop, resale shop, antique shop, quilt shop, hearing aid store, Garrettsville Foot & Ankle Clinic (Drs. Frank Stoddard and Michael Maschek), and some offices.

Record-breaking DFCon14 Crashes Servers, Makes Impact, Worldwide
The 14th International Symposium on the Diabetic Foot (DFCon14) once again hosted delegates from some 50 nations, worldwide. "As ever, our community gathered together, and it is a global community," reported Dr. David G. Armstrong, Professor of Surgery and DFCon's co-founder and co-director. Technology was front and center more than ever in this conference. The servers carrying streaming video via portal education worldwide were overloaded, requiring on-the fly additions of back-up high-definition streaming video servers to the thousands watching live and on demand around the world.

Technology also played a more poignant and heart-warming role. Professor Michael Edmonds of King's College, London, who took ill minutes before boarding his transatlantic flight, was able to accept the Edward James Olmos Award from a King's College Hospital conference room. He was introduced both by Mr. Olmos, who was in Australia filming a movie and by Rick Dees, world-renowned top-40 disc jockey.

Robert Parker: It is so much more sophisticated today but, in reality, it comes down to relationships; it's just relationships. I remember my first presentation about creating a new residency program. Many doctors got loud and were asking if we were going to be giving out a DPM degree. They did not know much about our profession. One general surgeon (whom I knew socially) stood up and said, gentlemen, let's be gentlemen. I left there thinking that there was no way they would allow this residency, but soon after, they said yes, as they realized the financial upside to having us, and that was the beginning of the birth of the Harris County residency program.

Try cuprum metallicum, 3 pellets taken sublingually at the time of the cramp.
I've had great success having patients add a magnesium supplement later in the day and evening. Make sure they are purchasing a quality supplement that is a GMP-certified (Good Manufacturing Processing) supplement, preferably 100 mg tablets for ease-of-use. My starting protocol is 200 mg at dinner and 300 mg before bed plus calcium, since they work in tandem. If this is not effective, have the patient increase Mg++ by 100 mg and then continue this dose once results are achieved. It is difficult to overdose on magnesium since 80% of Americans are deficient, however, the limit is to bowel tolerance (very loose stools).

RE: Has Laser Nail Fungus Removal Been Proven Effective? (Elliot Udell, DPM)

Dr. Udell raises a very legitimate concern. When the NY Times comes out with a position, people take notice. I am interested to hear if anyone has information on the specifics of the in-vivo results of the October 2013 article in the Journal of American Academy of Dermatology by Carney, et al. which was mentioned. Media information is all over the map. Not long ago, Consumer Reports put out a number of 90% effective treatment of laser on toenails with onychomycosis. The literature is also all over the map. Noguchi in 2013 reported a 27% decrease in turbidity, which was much less than many industry-backed studies. I look forward to having the profession continue to wade through this complicated data comparison to develop a reasonable consensus for how effective the laser is.
As just one small anecdotal data point, I have had lasers help my own nails, and some of my patients and I have had failures. I have come to my own conclusion that much of the success is driven by the physiology of the nail bed and immune status of the patient you are treating .

I have been using a laser for treatment of mycotic toenails for close to three years. I have found it to be an effective modality. Does it cure all? No. You will see cures in some and improvements in many, if not all. Recently, I have been using it in combination with Genadur liquid.
None of my patients ever reacted like the one in the previous posting. They realize it is part of a treatment plan which includes improved pedal hygiene and prevention of tinea. You don't have to use a laser, you can use oral medication if you wish, or use laser with a topical. Use something. Bi-monthly debridement by itself is not a treatment plan.

One thing to keep in mind: if a concierge patient pays an annual or other fee to obtain the benefits of concierge services is that providers who care for Medicare patients must stay within the Medicare assignment. This includes limiting charge rules of what can be collected from the patient for each covered service. Anyone considering the concierge model of practice should work closely with a healthcare attorney to make sure the model complies with federal and other regulations.

RE: Should the DPM Degree be Reduced to Three Years (Robert Creighton, DPM)

McMaster University in Hamilton, Ontario Canada offers a three-year program in medicine that uses a problem-based approach to learning. The program operates on an 11 month-a-year basis.
I graduated from CCPM in 1977 after the three-year curriculum. As I understand it, the experiment in 18 medical schools and one college of podiatric medicine of the mid 1970s was instituted in order to provide more doctors for our country. The fallacy in this equation was other than the one double-size graduating class of 1976; the United States still only realized ONE graduating class of doctors from each school, each year. We just had a little less temporal experience than our 4-year brethren.
Because this program made little sense and yielded no positive overall benefit, these programs were abandoned. Simple explanation. Forget the financial self-interest issues of the schools involved. I believe they had little overall input in the matter. The governmental gurus made another mathematical error.

RE: The Passing of Keith Springer
As a 1993 graduate of NYCPM, I was very sad to hear of the passing of Keith. Along with other members of the surgery department at NYCPM, Keith always made the learning process fun. As an educator and clinician, Keith was second to none. NYCPM, along with podiatry, has lost a really great guy. My thoughts and prayers are with Keith and his family.
