OBITUARIES
Gordon Duggar, DPM, 83
 
Dr. Gordon Eugene Duggar, died Sunday from complications of a previously occurring stroke. He was 83. After graduating from podiatry school, Duggar opened a practice in Decatur, until he was asked to join the staff at the Atlanta Veterans Administration Hospital in 1986. While there, Duggar earned awards for his dedication such as the Secretary for Veterans Affairs “Hands and Hearts Award.” He served as the chief of several programs and boards.

In the early 1990s, Duggar created the VAMC Podiatric Residency Program, a 36-month program aimed at educating and training well-rounded podiatric physicians, said longtime friend and VA Hospital podiatric surgeon, Dr. A. Louis Jimenez. Throughout his career in podiatry, Duggar had been dedicated to the improvement of the field, often attempting to introduce bills in the legislature that would help to eliminate “insurance and education issues for podiatric surgeons,” Jimenez said.

The American Academy of Podiatric Sports Medicine has announced that Magali “Maggie” Fournier, DPM from Onalaska, Wisconsin has joined the ranks as the newest Fellow of the AAPSM. Maggie joins the other 161 AAPSM Fellows who have successfully passed a written examination and oral interview. Maggie was presented her certificate at the AAPSM Membership meeting held last weekend in Tampa, Florida.

UK Podiatrists Search for a Great North Runner for Children’s Charity
 
The Bupa Great North Run is firmly established as the world’s greatest half marathon. The event has now grown to a record 56,000 accepted entrants from over 122,000 applicants in 2013. Gosforth-based podiatrists, Howlett & Dickinson, specialize in biomechanics and gait analysis for runners and those about to embark on running for the first time. They have teamed up with Start Fitness, Newcastle to search for a runner for 2014’s Great North Run to run for local children’s charity, Smile for Life. Smile for Life is a charity dedicated to enhancing and enriching the lives of disadvantaged and disabled children.

Podiatrist Judith Dickinson says, "There are thousands of people running or thinking of starting running, and we wanted to give one person the chance to run the Great North Run this year on us. As a surgeon of 34 years and a former residency director who evaluated his attendings' procedures weekly with his residents, each surgeon has his/her signature and you can differentiate the few technically good surgeons from the rush jobs. There is a difference. I am in no way or manner equating time to complete a surgery with fees. I do think 5 minutes is ludicrous to do a hammertoe procedure. Dr. Kornfeld, in a rare moment for him, is promoting poor advice for the profession. I can assure you that using prudent care, proper tissue handling, bone handling and rasping, closure and biomechanical analysis takes more than 5 minutes.  
 
By the way, I hope you bring pre-op x-rays which many surgeons do not bring to the OR. Surgeons devoting 10 minutes, and for sure 5 minutes, on a surgical procedure are doing their patients a disservice. You cannot do a good job in 5 minutes for certain, in my opinion. I'd love to be the lawyer who reviews this 5 minute case if a patient has a poor outcome. What's the rush anyway? It takes a few extra minutes, maybe 10 extra minutes, to do the procedure and check that no problems or bone spicules exist before sending this patient his/her merry way home.

I appreciated the comments from Dr. Goldman regarding the crazy charges in New York for toe surgery. His main point was that we should all provide a real cost estimate for our services so that our patients are able to realize what they are getting and at what price they are getting it. At the same time, I couldn’t help but see that we as providers are also keeping our heads buried in the sand, much as the patients who do not realize real costs.
 
How many times have you heard a representative for a drug company or surgical equipment company say, “It will only cost your patient “X” dollars” or “Don’t worry, the cost of the implant will be paid for by the insurance company.” When was the last time you asked your rep selling compounded formulas, “How much does this really cost?” or asked your surgical equipment rep, “How much does this small 16mm cannulated 2.7 screw really cost?” Have you ever seen the actual bill to your surgical facility? It would probably open a few eyes!

While I rarely get surprised by anything these days, I have to admit that this unfortunate story left me speechless. This exemplifies the hypocrisy of our healthcare system, and the selfishness by some trying to stuff their pockets, regardless of the repercussions. I am not only referring to some provider’s greed, but the insurance carrier’s greed that may sometimes cause the provider to rationalize the over-the-top behavior as some sort of revenge. 
 
The provider-insurance carrier relationship has frequently been adversarial. Ridiculously low reimbursements, policy changes, and restrictions have sometimes been met with...
