CA Podiatrist Appointed to Podiatric Medical Board of California
 
Governor Gavin Newsom recently appointed Daniel Lee, DPM, PhD of Sacramento to the Podiatric Medical Board of California. Dr. Lee earned a Doctor of Podiatric Medicine degree from the California College of Podiatric Medicine and a Doctor of Philosophy degree in biomedical sciences from Chulalongkorn University. He completed his post-graduate residency training at the Los Angeles County/University of Southern California Medical Center.

Dr. Lee also completed fellowship trainings at the National Institutes of Health, Bethesda, MD in biomedical sciences; at the Schulthess Klinik, Zürich, Switzerland, in orthopaedic surgery; and at the Russian Ilizarov Scientific Center for Restorative Traumatology and Orthopedics, Kurgan, Siberia, Russia. He is Board certified in foot and ankle surgery by the American Board of Foot and Ankle Surgery and has been a clinical professor at the California Northstate University, College of Medicine since 2013.

In any work environment, tensions arise between staff members and managers. The trick, however, is learning to manage that conflict so that it does not escalate into a full-blown crisis.
 
2.  Involve an Unbiased Party
 
Ask the conflicting parties to meet individually with an unbiased party, such as your practice's HR manager, an office manager, and/or your managing physician partner. Finding a neutral third party is critical, especially if a physician is involved in the conflict, Beth Brascugli De Lima, SHPR-CA, principal and founder of human resources consulting firm HRM Consulting, Inc. told Physicians Practice

Query: Source for Repair of Micromat 3000 Drill
 
Does anyone know of a nail drill called Micromat 3000 (it's a drill with a spray)? It was distributed by the MED-PRO Corporation. It appears that MED-PRO has just disappeared, as their phones are no longer in service. A web search does not show who can service these nail drills.

I currently use Doxy.me for telemedicine visits. My concern is the video recording of this session in a HIPAA-compliant manner. Replay Media Catcher is something I am considering using so long as the recording is saved on my computer and not anyone else's cloud. What tools are other podiatrists using to record telemedicine visits?

RE: Varicella Zoster Virus Presenting as Lower Extremity Ulcers and an Atypical Myeloid Infiltrate

Cutaneous manifestations of varicella zoster virus (VZV) include diverse clinicopathologic presentations, particularly in immunocompromised patients and have the potential of mimicking various cutaneous disorders. A 75-year-old man with a 35-year history of Crohn's disease treated with methotrexate is described with VZV presenting with atypical clinical and histopathologic features suggesting a leukemic infiltrate but no occult myeloproliferative disorder. He presented with a few months of progressive ulcerations of the lower portion of the left extremity and new ipsilateral palpable purpura as well as a history of left foot osteomyelitis resulting in toe amputations and macrocytic anemia of unknown etiology.

Physical examination revealed large ovoid ulcers with eschars of the posterolateral aspect of the left leg and dorsal aspect of the foot and palpable purpura extending proximally to the knee and concurrent cribriform ulcerations coalesced over the left ankle and dorsal aspect of the foot (Massoud, CM, et al. Journal of the American Academy of Dermatology, Jan 2021.

RESPONSES/COMMENTS (CLINICAL) - PART 1A

This is a dilemma I have faced many times across the entire span of age groups. It is a difficult presentation, which I have treated multiple ways with varying results. Taking x-rays to rule out subungual exostosis is a place to start, although I find radiographs can be misleading. Surgically, I have found these to have a cartilaginous cap, such as BPOP (Nora's lesions) and osteochondromas, and therefore are not fully appreciated on radiographs. Likewise, lack of skeletal maturity in an 8-year-old would make this even more difficult to appreciate.  
 
I wonder if any of my colleagues have used ultrasound to help detect these better than radiographs. I've tried "fish mouth" excision on occasion and have had limited success. I'd recommend removal of subungual exostosis, even if a true exostosis is not present, such as removal of a portion of the distal tuft of the phalanx during the same surgical setting. I find the KT tape example to be a fascinating creative idea; kudos to this example. Bottom line is one size does not fit all. I may consider in the future trying KT Tape post-surgically, if unsuccessful.

I would try placing two strips of Onyfix on that toe proximally and distally to maximize the support and lift. Then I would take my blacks file and make sure to regularly clear space at the distal portion of the nail until the free edge of the nail grows over the tuft.
