Air Force Major Named Podiatrist of the Year on AMC Level

In a message from Gen. Raymond E. Johns Jr., AMC commander, DGMC officer Major Andrew Puckett, DPM was recognized as the best in the command for fiscal year 2010 as USAF Podiatrist of the Year. As a recipient, he is authorized to wear the Air Force Recognition Ribbon and will now compete at the Air Force level.

Dr. Puckett graduated from the Ohio College of Podiatric Medicine in 2000, and completed residencies at Metropolitan Hospital in Des Moines, IA and Clarion University of Pennsylvania. After being in private practice in Pennsylvania, he entered the Air Force in 2007. He is currently Chief of Orthotic Services and Supervises performance and training of a 30 member team.

AAPPM Members "Get It” - $13,338 Raised at Seminar for APMAPAC

Attendees at the AAPPM annual Million Dollar Summit, held in Ft Lauderdale, Florida in January 2011 raised $13,338 toward APMAPAC. Each year AAPPM President, Jeffrey Frederick, DPM suggests to those attending to make a contribution to APMAPAC. "Getting 100% participation by attendees has become a tradition that the AAPPM is proud of," stated AAPPM Chairman, Hal Ornstein, DPM.

"This is the only podiatry seminar that I am aware of that has been able to do this, and we will continue to do so. Barefoot Running Shoes Can be Harmful to Feet: NY Podiatrist

Barefoot running shoes have been gaining popularity in recent months, as they are designed to re-create the natural sensation of running "barefoot" on man-made surfaces like concrete and asphalt. Dr. Robert A. Kornfeld, Founder of the Institute for Integrative Podiatric Medicine, wrote for the Huffington Post that barefoot running shoe manufacturers believe that "the human foot, unimpeded by synthetic surfaces and restrictive running shoes, should function at its best."

"Choosing to run on non-yielding surfaces without the protection afforded by proper running shoes can be harmful to the foot and ankle and cause even more problems downstream from compensation patterns," Dr. Kornfeld wrote.

NYCPM, SNPMA, and NYCPM Pediatrics Club Co-Host Toy Drive for Harlem Tots  

A group of NYCPM students, Class of 2012, demonstrated their holiday spirit recently by collecting and donating toys to children at a nearby shelter for homeless families. The students ran a toy drive at the College, co-hosted by the Student National Podiatric Medical Association (SNPMA) and the NYCPM Pediatrics Club during the first weeks of December, collecting more than 75 toys.

The students who were key to the Toy Drive were Alysaa Phyars, President of SNPMA, Caroline Mpande, VP of SNPMA, Janale Beckford, Secretary of SNPMA, Jennifer Maungu, Treasurer of SNPMA, and Sherie Neufville, President of the Pediatrics Club. Faculty members Eunice V. Ramsey-Parker, DPM, MPH, Associate Professor of Podiatric Medicine, and Clinic Administrator, and Laurence J. Lowy, DPM, Professor of Pediatrics, and Dean for Student Services, as well as William H. Graham, VP, Information Technology & Operations, were also critical to the success of the Toy Drive.

QUERIES (CLINICAL)

Query: Overlapping 2nd Toe

This patient presented to my office for care of a painful hammertoe. She remembers an accident with her foot a few years ago, and ever since then, she has had the hammertoe. Her medical history is uneventful, except for 30 years of smoking.

Overlapping 2nd Toe

Examination revealed that she has a reducible hammertoe and 5/5 muscle strength for the extensors as well as the flexors. What is the best procedure in fixing this condition?

I agree with the prior postings that there may be a possible adjacent space neuroma. If you are re-doing the procedure, take the proximal stump and bury it in the muscle belly. One final thought - rule out tarsal tunnel syndrome, which may have manifested in the same location as a typical neuroma.

Dr. Young asks, "what can we do" to stimulate an immune response against the HP virus? He claims he has only witnessed this phenomenon twice in 25 years treating pedal verrucae. I must conclude he's likely never tried cimetidine, nor any other immune system stimulating therapy against the HP virus. The "anecdotal lack of EBM" whine has now become a very weak and lame excuse not to utilize what is clinically effective and safe. Ask your colleagues across the country, who have finally opened up their minds and tried cimetidine, as just one immune system stimulating therapeutic example, what their clinical response has been.

No, it's not 100% effective...but, what...

RE: Credit Cards on File

My small practice has seen a significant improvement in the reduction of accounts receivables by using a company that puts credit cards on file. We currently require most of our patients to give this information at the time of intake. We do make exceptions for Medicare/Medicaid and other cases where we know that we will receive full payment. Although we do lose a few patients who refuse, we think it is better to have a guarantee of payment than simply a hope and prayer that they will eventually pay.
 
Furthermore, we spend far less time on the phone doing deductible or verification work as we believe that the bill is ultimately the responsibility of the patient. At the end of the day, insurance companies are passing more medical expenses back to the patient. We do not think that practices will be viable in the future unless they are able to get patients to pay their portion. We also find that getting paid the vast majority of time makes it easier for us to afford the pro-bono work that we all wish to do.

I have come across a very affordable electronic medical records program. The program is called Amazing Charts. The program has the following features. The initial course is only $1,995. The second-year, it costs $995. This includes ,ePrescribing. CCHIT-certified, same-day support, office messaging and reporting. Chart entries can be made by typing, templates which you can make by yourself, or Dragon medical speech recognition. You can also use MS speech recognition. There is also a very robust user forum. This may be a very good program for those who are not afraid of making up their own templates.

The MGMA position lists seven important parameters for a medical practice. I assume that means that we should dedicate time, energy and money in their direction in order to “succeed” in practice at the expense of other practice variables: 1. accounts receivables and collections 2. patient (customer) satisfaction 3. productivity 4. capacity 5. staffing 6. profitability 7. cost management.
 
It looks to me like they are talking about my friend who has a $2,000,000-a-year lingerie boutique.

For me, the list has at the top: 1. diagnostic acumen 2. treatment protocols 3. patient rapport

Where did we go wrong? Hint… HMOitis. How do we repair and re-acculturate?

RE: Coding a Winograd (Tony Poggio, DPM)

I believe that Dr. Poggio’s, response, to use CPT 11750 as the code for a Winograd is acceptable, but code CPT 11765 fits the description much better. 11750 does not consider any wedge resection of skin, and 11765 can be performed without touching the nail plate. Most commonly though, the Winograd surgery for ingrown nails includes a wedge resection incorporating both hypertrophic skin and nail plate.
