Improvements in Patient Outcome Measures Found with Ankle Procedures
 
Despite similar improvements in comfort and function after ankle arthrodesis or total ankle replacement, results published in The Journal of Bone and Joint Surgery showed total ankle replacement performed with newer generation implants had better patient-reported outcomes.
 
“This prospective, multicenter cohort trial comparing patient-reported outcomes from ankle replacement and ankle arthrodesis showed clinically significant improvement with both treatments, with better patient-reported outcomes and pain scores in the group treated with ankle replacement at 3-years post-op,” Bruce J. Sangeorzan, MD, told Healio.com/Orthopedics.

Essential Provisions to Include in EHR Contracts

As more healthcare providers are considering potential changes in electronic health records, it's time to take a fresh look at the contracts they are signing. Providers need to insert more protections in their legal documents as they make decisions to change vendors.

In contracts, "I put in that the vendor is required to meet and maintain the product to continue to meet standards," says Corinne Smith who has worked for Seton Healthcare Family and UT Health Center San Antonio with complex healthcare interactions and contracting. "I'm often surprised at how few contracts have ONC certification stipulations in them. Similarly, there needs to be an obligation in the contract requiring the vendor to update software to meet regulatory changes.

Medical Practice Financial Reports Docs Need to See - Part 10
 
Hard data is crucial to Kyle Matthews, chief executive officer of a cardiology practice near Phoenix, and he regularly shares them with his physicians through 10 key financial reports. The numbers tell him — and them — when to hire and when to fire, who has too many appointments, and who has too few, and even when it's time to investigate missing referrals. Matthews recommended providing these financial reports to partner physicians:

Matthews uses this monthly report to analyze how well physicians are filling available appointment slots. It also assists him in figuring out whether it's time to hire more physicians or adjust scheduling.

Can CPT 15002 (surgical preparation or creation of recipient site by excision of open wounds, burn eschar, or scar or incisional release of scar contracture, trunk, arms, legs: first 100 sq cm) be used for wound prep when performing a split-thickness skin graft (STSG), or is this just a scar tissue release code?

Response: In my opinion, CPT 15002 can be used for wound prep when preparing a site to receive split thickness skin graft.
 
CPT says these codes are for “services related to preparing a clean and viable wound surface for placement of an autograft, flap, skin substitute graft or for negative pressure wound therapy”.

I have been employed in an academic vascular surgery group for the past 10 years. Vascular surgeons do a lot of work with peripheral vascular disease, and depending on where you practice, do most of the amputations for non-reconstructable PAD. I was hired as an NP (I am dually credentialed), but they quickly realized the potential for having a podiatric surgeon in the group, so added me on to the staff with surgical privileges. You will end up doing a lot of limb salvage. If your state permits, they will have you do all the amputations up to the ankle.
 
I found it to be very interesting work and introduced all of the 'toe' aspects of 'toe and flow' to the team. I did the reconstructions, grafts, etc., and did all the follow-up in clinic. I introduced advanced wound care products, TCC, orthotics, etc. to the group and assisted in training their resident team in limb salvage principles. You will learn a lot about the value of vascular reconstruction, non-invasive vascular testing, etc. 
 
Make sure your vascular surgeon does distal bypasses and distal angio work. Like I said, it was very rewarding work, but you need to be sure to negotiate your role in the practice.  A podiatrist will never be replaced by an NP/PA. We have 11 of them, but they can't do the surgeries and don't have the podiatric training to manage post-op salvage to ambulation. Podiatric care is essential to a full scope vascular surgical practice.

I have two cousins who many years ago became dentists. Today, there are individuals called "denturists" in some states who can take dental impressions and make dentures without a dental referral. In a couple of states, dental hygienists can practice privately, and in one state can even fill a tooth without a dentist's supervision. In a number of states, a certified nurse anesthetist may practice independently without medical supervision. Also, in some states, a physical therapist does not have to have a doctor's referral to see a patient.
 
Any person rendering a service that can be construed as the practice of medicine will be held to a medical standard of care while so doing. This is true in all of the jurisdictions that I am familiar with. I still say it is more important that a podiatrist be defined as a physician than that he or she be granted an unlimited scope of practice. You do not find orthopedic surgeons delivering babies or ophthalmologists performing bunion surgery, even though it is in their technical scope of practice. They are limited primarily because of liability issues.
