IA Podiatrist Uses Less Invasive Implant for Arthritic Hallux Joints
 
“There’s an implant (Cartiva) placed into the big toe joint. It mimics actual cartilage and it’s used for people who have bad arthritis in their big toe joint without a bunion. The procedure involves an incision on the top of the big toe and the tendon is then moved out of the way and the arthritis on top of the big toe is removed. And then there’s usually a cartilage defect in the center of the big toe joint which is then reamed out and the implant is then placed in the first metatarsal head,” said Dr. Brian Dix, podiatrist with Avera Medical Group Orthopedics and Sports Medicine in Sioux Falls.

This serves as an alternative to ‘big toe fusion.’ “And with a big toe fusion, the big toe is fused and never moves again. And traditionally you have to stay off that for six to eight weeks before you can walk,” said Dr. Dix. Implanting the Cartiva is less invasive and recovery is substantially less.

John Taylor, DPM and Alma Velasquez, DPM will join El Paso Specialty Hospital on Jan. 2. Dr. Taylor is a graduate of the Ohio College of Podiatric Medicine and completed his residencies at South General Hospital in El Paso and St. Michael Hospital in Cleveland, Ohio. He is board certified with the American Podiatric Medical Association and the American Board of Foot and Ankle Surgery.

Dr. Velasquez is a graduate at the Barry University School of Podiatric Medicine and completed her residencies at West Palm Beach VA Hospital in Florida and Southwestern Hospital in El Paso. She is board certified by the American College of Foot and Ankle Surgeons.

RE: Joint Venture with Vascular Surgeon

From my practical experience, vascular surgeons, as with the rest of the MD community, will typically employ a physician assistant (PA) . Therefore, I suspect that you will be taking your time training a PA about what we know until the vascular surgeon replaces you with the PA. The solution may be to have a five (5) year contract with some clause on what money you will receive if there is termination for any reason at all (or for no reason at all) short of the time contracted.

Every time I hear discussions about nurses taking over podiatry, I think of how wayyy-back-when dentists said to each other "hummm, this may happen." The difference between them and podiatrists is the dentists organized to make certain it didn't. Thus, was borne the dental hygienist (the legislation snatched it away from the potential of nurses doing this work without the dentists) who does the work the dentists do not want to do, are educated to do it correctly, and are legally the only ones who can (other than dentists). But these dental hygienists CANNOT under legislative law do so in any state (that I know of) except under the direct supervision of a dentist. And it was all put through by the state dental associations. I have discussed this with many podiatrists over the years, even suggested organizing and getting the restriction developed, but they never got it!! So, now, it is too late. 
 
Think about this: no patient can have their teeth cleaned, have x-rays, etc. EXCEPT in a dental office or clinic - wherever, they must have a dentist on staff. And the patients have no choice of where to have this work performed. Wouldn't you love that? But alas, you are too late, I fear.
