PODIATRISTS AND SPORTS MEDICINE
AK Podiatrist Discusses Winter Running

Manx Qualye, DPM, podiatrist at Aurora Foot and Ankle, says "The big problem in the winter is if your joints get cold then that can create problems with the synovial fluid, because the joint is not going to function as well when it is cold and it can create cartilage issues and arthritis with the joint being too cold and you are trying to push through. You want to keep your joints as warm as possible."

Dr. Manx Quayle

There are also shoes designed for running in the winter. Some have cleat-like spikes on the bottom to aid in grip if someone were to be running in a place that might have some patches of ice. Most all shoes designed for winter running also have a harder sole. Dr. Quayle recommends finding a running shoe that is best tailored to your needs, which can be accomplished at most sports stores.

PODIATRISTS IN THE COMMUNITY
NH Podiatrist Kicks Off Shoe, Boot, Sock Drive

A local podiatrist has kicked off her annual holiday shoe, boot, and sock drive to help local individuals in need. Every year, Dr. Billie Bondar has run the drive as a way to reduce the number of local adults and children who may otherwise go without adequate footwear and snow gear during a pivotal time of the year.

Dr. Billie Bondar

Through the efforts of Bondar’s office, patients, community members, local shoe stores, and numerous other area businesses that contribute to the cause, Bondar said each year of the drive directs “hundreds and hundreds” of shoes and boots to local homeless shelters and social service organizations. “I see feet all day,” she said. “This is important to me.”

PRACTICE MANAGEMENT TIP OF THE DAY
Understanding Your Practice's Bottom Line - Part 4

Let's take a look at four steps you can take to help you understand what is driving the bottom line at your practice.

4) Connect the dots

Keep in mind that each decision you make should comply with your group's financial, strategic, and patient care objectives. Therefore, your group should have a decision framework in place that provides an efficient process for making business decisions. For example: Who has authority to decide? What are the financial and shareholder compensation implications? Is there a downside/upside? How soon must a decision be made?

Medical practices are unique businesses, and as an owner or administrator of this unique business, you may be well-served to seek revenue cycle management services, medical practice management services and/or, financial management advice from third-party experts aligned to the field.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Nationwide Shortage of Local Anesthetics Mixed with Epinephrine
From: Robert D.Teitelbaum, DPM

For reasons not explained to me nor anyone else, as far as I know, there is a shortage or absence of local anesthetics mixed with epinephrine. I have found an easy and I believe safe solution to this problem. Adrenaline/epinephrine is available from our suppliers (you may have an extra vial in your emergency kit) for less than $10 for a 1mg total in 1 cc, at a 1/1000 ratio. I extract 0.2 cc from this vial and inject it into a 50 ml vial of bupivcaine 0.5% for a 1/250,000 ratio. I repeated this four more times to use up that one vial and now have 250 ml of local anesthetic that should last until the shortage is over---which the suppliers say is close at hand.

I have used this combination twenty times in the last two weeks without any side-effects or sequelae. The blanching effect is the same and is very appreciated when one needs a bloodless field, especially in the short-term.

RESPONSES/COMMENTS (NEWS STORIES) - PART 1
From: Richard Mann, DPM

Tom DeLauro represents the very best of our profession. He is a brilliant clinician, surgeon, and educator. He has been an exemplary role model for all who have known him and those of us fortunate enough to have been trained by him. It is with great affection and respect that I wish him the very best on his retirement.

RESPONSES/COMMENTS (NEWS STORIES) - PART 2
From: Charles Lombardi, DPM

A point of clarification from Dr. Rogers’ note. If a fellowship is approved by our accrediting agency (CPME), then and only then, they may have available funds, so a CAQ is unnecessary. Also, there is some confusion as to funding of fellowships. They are generally paid at half the direct funding of the program and all of the indirect costs, but there is a caveat. I cannot find in the Federal Register that other than geriatric and community health, fellowships are reimbursable, although speaking with my allopathic colleagues, I learned that they are getting reimbursed in other specialties. The one thing that is for sure is that fellowships approved by non-accredited agencies in the eyes of the federal government are definitely not reimbursed.
