NYCPM Professor and Chair of Medical Sciences to Retire
 
Professor and Chair of Medical Sciences, Thomas M. DeLauro, DPM, will retire at the end of December. Valedictorian of the Class of 1976 and a magna cum laude graduate of NYCPM, Dr. DeLauro divided his two-year residency between Peninsula Hospital Center (Far Rockaway) and Atlanta Hospital (Georgia). He has worked at NYCPM in different capacities since 1978.

A full-time faculty member, he was Academic Dean from 1980 to 1992 and had earned full professor rank in both Medical Sciences and Surgical Sciences. Dr. DeLauro worked at the College part-time after that year until 2007, at which time he regained a full-time position at NYCPM. He was also editor-in-chief of JAPMA from 1987-1990. Dr. DeLauro maintains two private practices, one in Staten Island and the other in Manhattan; he shares the Staten Island practice with his daughter, Nicole DeLauro, DPM, a 2006 graduate of NYCPM.

NJ Podiatrist Joins the Center for Musculoskeletal Disorders
 
Daniel Popowitz, DPM, a specialist in foot and ankle orthopedics, has joined the Center for Musculoskeletal Disorders (CMD). Dr. Popowitz graduated from the New York College of Podiatric Medicine in New York City, followed by a three-year comprehensive foot and ankle surgical residency at Montefiore Medical Center, University Hospital for the Albert Einstein College of Medicine.

During his residency, Popowitz was a student of some of the most highly regarded pediatric orthopedists and foot and ankle orthopedists in the country, as well as top orthopedic trauma specialists at Jacobi Medical Center. He is an associate of the American College of Foot and Ankle Surgeons. His publications include articles on bunion deformities and Achilles tendinitis.

Two Bridges, by the East River in Manhattan, may be tiny. But massive developments often seem to wash up there. This middle-class area, which has never really had its turn as a fashionable neighborhood, could seem like a land the City forgot.

The Waning World of the Solo Practice - Part 3
 
Apart from the obvious solution of joining a group practice or selling to a hospital to become an employee, what is a solo practitioner to do? In some cases, hiring trained, mid-level providers as associates can increase patient revenue without significantly increasing cost. For those solo practitioners who are located in affluent communities where patients are willing to pay out-of-pocket for greater access, a growing option is to convert to a "concierge" medical practice (also known as "retainer," "boutique" or "membership" medicine).
 
In these types of practices, an annual fee ensures same day service, 24/7 telephone access to the physician, and enhanced appointments of longer duration. While I was aware of these types of practices for internal medicine and family practice, I was surprised to learn of their existence in the specialty of dermatology. I was able to find concierge dermatologic practices in six medium and large cities around the U.S. with little difficulty, including one here in Tucson, Arizona. The board-certified dermatologist even operates a mobile service, and is willing to "come to your office or home" for an appointment!

Understanding Your Practice's Bottom Line - Part 3
 
Let's take a look at four steps you can take to help you understand what is driving the bottom line at your practice.
 
3) Start with accurate documentation
 
Although claim denials may end up in the business office, they often start in the physician's office due to a lack of required, proper documentation. As an independent business owner, you need to stay up to date on proper documentation methods and continuous coding changes to lessen the likelihood of payment denials or long accounts receivable (AR) cycles. Identifying trends in front-end denials should lead to corrective training, education, and action. Similarly, practices must maintain and regularly review their payer contracts to ensure payer compliance and market rates are in place.

Dr. William Grady Dunn Sr., passed away on December 5, 2017. He graduated from the Ohio College of Podiatry with honors in Cleveland, OH. He received special recognition for having served six years on the NC State Board of Podiatry. In addition, he served as president of the NC Podiatry Society and served on the Board of the NC Cancer Society.

Query: Patient Seen in Office While Residing in SNF
 
We have a patient who was seen in the office for a post-operative visit. The patient had resided in a skilled nursing facility (SNF) for two weeks at the time of the visit. His wife brought him to the visit. We took an x-ray in the office, billed Novitis Medicare, CPT 73630 (1 unit). Medicare is saying they overpaid the claim and want to take back what they paid. The reason given: “Services within a Skilled Nursing Facility are subject to consolidated billing”.
 
The representative is telling me we have to bill the nursing facility, they bill Medicare and then pay us. She cites the 2017 CMS site for guidelines on Consolidated Billing. CPT 73630 is not on this list of procedures that can be billed by the doctor’s office. This just doesn’t make sense. Has anyone else run into this?

Response: The rep was only partially right. They inappropriately only overpaid you for the technical portion of the x-ray study allowance. X-ray studies (the technical component) fall under Consolidated Billing, but you can bill and get paid for the x-ray study professional component from your Medicare Part B contractor (Novitas) by applying a “26” to the x-ray code.
 
The skilled nursing facility (SNF) is paid in a lump sum for services such as the x-ray study technical component. If you have a prior arrangement with the SNF, you may get paid directly by the SNF for the technical component. If you haven’t set-up an agreement with the SNF, contact their administration. The worst that can happen is that you won’t get that portion of the study…but you’ll know for the future.

RE: CRIP Scheduling

As a residency director for 20 years, my program as well as many other programs from the tristate area have benefitted from a local CRIP facilitated by NYCPM in the week following the national CRIP. I in no way wish to take away from the national CRIP. It serves a valuable purpose. However, having a local opportunity to interview greatly benefits students and programs who for a variety of good and valid reasons cannot travel to Texas.
 
This year, the local CRIP would conflict with the annual New York State meeting. Plans were in place to hold the local CRIP the weekend of January 27 and 28. A request was made to the AACPM board to push back the closing date by 4 days. Such a move would not negatively hurt anyone, would not cost any money, and would benefit many. Unfortunately, the request was denied because of one single negative vote by a single AACPM board member. Their bylaws require a unanimous vote. Consequently, many will suffer. We do seem to eat our young.
