TN Podiatrist Discusses Ingrown Nails
 
"The configuration of the nail borders on some people's toes are significantly curved inward and as such, the nail has a predisposition to becoming ingrown," says Mark Hinkes, DPM, a podiatrist at Veterans Affairs Medical Center in Nashville, TN, who is board-certified in foot surgery and wound care. "In this situation, the person is confronted with the nail borders being tender and just about to become ingrown." This situation makes wearing closed shoes painful and can cause functional disability, preventing people from persuing their normal activities.

Many people elect to try over-the-counter medications. "These products usually contain salicylic acid, a chemical which destroys human tissue painlessly. On a temporary basis, using medications like this may relieve the symptoms of the problem, but it will not eliminate the problem," says Hinkes.

Citi Bike Racks Block Access to NY Podiatrist's Office
 
New York City’s Citi Bike program is popular. A lot of riders love getting around town by bike. But the location of one Citi Bike rack is a big problem for a podiatrist in Greenpoint, Brooklyn.  The rack is right in front of his office and patients are having a tough time getting in. “It’s really the wheelchair patients. The patients with the walkers, those are the ones that are having a hard time,” said Dr. Vincent Meliso. "We’re more concerned with them falling more than anything else.”

The Citi Bike program is administered by a company called Motivate. But Motivate says the rack location is purely up to the NYC Dept. of Transportation. Dr. Meliso says he has contacted his community board and city councilman. They’ve written letters. But so far, no action.

Doctor Who Doesn't Use Computer Can't Regain License
 
A New Hampshire judge has denied an 84-year-old doctor's request to regain her license to practice, which she had surrendered partly over her inability to use a computer.
 
The state challenged Dr. Anna Konopka's record-keeping, prescribing practices, and medical decision-making. It said her limited computer skills prevent her from using the state's mandatory electronic drug-monitoring program, which requires prescribers of opioids to register in an effort to reduce overdoses.

Medical Malpractice Liability in the EHR - Part 3
 
In a recent article in Healthcare IT News, a major liability carrier in a study details the legal risks of the EHR, some of which we could anticipate and some of which were not so obvious. The EHR is creating some risk and liability for providers. Here are some takeaways of the study:
 
3. Copy and paste should be banned. EHRs make this easy, but it is one of the potentially most hazardous things you can do when you are charting electronically. It is lazy and propagates "ratty" data to the detriment of patient care. Insurers and the government have sophisticated tools to detect duplication, and you can be sure that legal counsel will use the same to show a lack of care and failure to accurately and completely document a visit. This can leave the provider open to liability for fraud as well as malpractice. Always document in your own words. There is no other safe way in which to chart.

UK Elective Surgery Ban for Smokers and Obese Patients
 
The committee that plans and oversees medical care for the county of Hertfordshire in England announced recently that unless obese patients lose a specified amount of weight and smokers quit smoking for at least 8 weeks, they will not be allowed to undergo elective surgery. Patients with a body mass index (BMI) greater than 40 must lose 15% of their weight within 9 months, and patients with a BMI over 30 must lose at least 10%. Free counseling for weight loss and smoking cessation is available to all.
 
Variations of these rules have been in effect in about one-third of England for quite a while. In some areas, similar rules have been waived if patients fail to meet their targets after a certain period of time. The Hertfordshire rules are unprecedented because they are in place indefinitely. The idea is that losing weight might decrease hospital lengths of stay and the incidence of complications after surgery and lead to better outcomes while saving money for the National Health Service (NHS).

There is an orthopedist in my city who routinely badmouths me and most other podiatrists directly to patients. He seems to focus much of it towards my patients in particular. He informs patients that their care or surgery was substandard, complications were the podiatrist's fault, etc. This has the effect of creating some really angry patients and nasty phone calls from them.  I am concerned that he is purposely trying to send a malpractice suit my way. An attorney told me that a phone call or letter from them would only likely inflame the situation. Any other ideas how I can resolve this?

I have been a member of both the ABFAS and ACFAS for the last 20 years. However, I have concerns about both organizations as it relates to the podiatric profession and the certifying foot and ankle surgical board exam. 
 
If the ACFAS has been touting that we are the "the leading experts in foot and ankle care" based on their recent PR campaign "Take a New Look at Foot and Ankle Surgeons", then they need to tell us how this is a fact if the passing rate is so low for the ABFAS board certifying exams.
