INTERNATIONAL PODIATRISTS IN THE NEWS
UK Podiatrist Offers Online Podiatry

An innovative new “concierge telemedicine” service is being launched by an entrepreneurial doctor in Dundee. Capitalizing on the growth industry that is online medical services, Dr. Bharti Rajput has created the first ever online podiatry service, aimed at increasing access and offering more time convenient consultations.

Dr. Bharti Rajput

She gradually became aware of a need for a better service for people with problems, and her “concierge telemedicine” concept was brought to life. “Patients often travel for three or four hours to see me, with problems that may be dermatological and could be solved with a foot expert’s visual assessment,” Dr. Rajput says. “Furthermore, when people have foot pain, they don’t automatically think 'I need to visit a podiatrist.' They will often go to the pharmacy and buy a footpad, and if it doesn’t improve, they might end up at their GP and be given some painkillers. An online podiatry service may provide the opportunity for those with foot problems to triage their medical concern and follow the next best steps to seeking a solution."

ON THE INTERNATIONAL LECTURE CIRCUIT

MD Podiatrist Presents Abstract at UK College of Podiatry Conference

The College of Podiatry hosted its annual conference in Liverpool U.K. November 16-18, with the theme “Keep Moving”. Over 1,500 health professionals attended this year's meeting representing 22 countries. U.S. podiatrist, Dr. Annette Joyce, was invited to present her short paper abstract “Biofilm and onychomycosis: An analysis of 8,816 toenail samples using qPCR and next gen sequencing”.

Dr. Annette Joyce

“The UK is struggling with two major threats to the future of podiatry,” according to keynote speaker, Professor Chris Nestor, University of Salford. “1. A lack of awareness as to what a podiatrist is and 2. Privatizing healthcare as a priority in the podiatry profession. The cost of University for a podiatrist could be 50,000 pounds for a 3-year degree, leading to historically low applications.”

MACRA UPDATE
Takeaways on the 2018 MACRA Final Rule - Part 10

10. Providers in areas affected by natural disasters during 2017 will receive a neutral payment adjustment in 2019. CMS adopted a policy to account for the recent hurricanes and other natural disasters during 2017. Per an interim final rule, providers in impacted regions are not required to submit 2017 MIPS data, and they can automatically avoid the 2019 penalty. Alternatively, they may choose to submit 2017 MIPS data to receive a MIPS score and MIPS payment adjustment based on category-by-category performance similar to other eligible clinicians.

CODINGLINE CORNER
Query: CPT 15275 and CPT 97597 Together

Can you bill CPT 15275 (application of skin substitute graft to face, scalp, eyelids, mouth, neck, ears, orbits, genitalia, hands, feet, and/or multiple digits, total wound surface area up to 100 sq cm; first 25 sq cm or less wound surface area) and CPT 97597 (selective debridement) together for the same area?

Also, if one CPT code is a Column 1 and the other is Column 2 of the CCI edits, are you allowed to bill them together?

Benjamin McKinney, DPM, Reidsville, NC

Response: There is a CCI edit between CPT 15275 and CPT 97597. Therefore, you should not bill for both if they are performed for the same ulcer at the same visit.

If one CPT code is Column 1 and the other is Column 2, you should not bill them together if they are being performed for the same pathology at the same location. For the most part, they should not be billed together unless one of them meets the requirements of the “25” (E/M) modifier or “59” (procedure) modifier.

RESPONSES/COMMENTS (OBITUARIES)

RE: The Passing of Stephen Winarick, DPM

On Halloween evening, my greatest longtime friend passed away, leaving a wonderful family — really my second family — and a wide array of friends and buddies: Dr. Stephen Winarick, a local podiatrist until his retirement 17 years ago. The hole left in so many hearts will ache with sadness forever.

Dr. Stephen Winarick

Steve was born and reared in the northeastern United States, arriving in Texas in the early 1970s as a podiatrist with the Air Force. He later purchased a podiatry practice in Waco and practiced here for nearly 25 years before retiring in 2000.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Standard of Care for Electric Debridement of Nails (Norm Wortzman, DPM)
From: Eliiot Udell, DPM

As an allergy sufferer, I had to make the decision many years ago to give up drilling mycotic, dystrophic nails or give up being a podiatrist. Even with expensive specially designed vacuum devices, I would still wheeze after grinding toenails. What lead me to my final decision to junk all of my Dremels and vacuum devices were papers published out of England showing that doctors who breathe in infected nail dust can develop pulmonary problems as a result.

There were also lectures at that time by respected colleagues on the OSHA regulations, where the experts said that if you do grind mycotic nails, the dust remains in the air for over 8 hours, and legally you cannot use that room for the rest of the day. This made sense to me. A more recent paper published out of Australia on March of 2015 in JAPMA by Hainsworth, et al., concluded: "The presence of viable fungal pathogens in the dust could potentially pose a health problem to podiatric physicians."

When I first gave up the odious practice of grinding infected toenails, I did lose some patients to colleagues who continued the practice. Most patients, however, were happy when I explained to them that breathing infected nail dust from other patients could be a detriment to their own well-being. In one case, I compromised and gave the patient an emery board and she was happy.

RESPONSES/COMMENTS (NEWS STORIES) - PART 2
From: Janet McCormick, MS

Texas has mandated that nail salons have autoclaves and thus began the charade of their use by many salons, such as putting implements in previously used pouches when nothing but a quick rinse was performed; there are many more "fake sterilization" practices. Also, there is the
problem of the low number of inspectors to keep up with the requirements. More laws and mandates do not work; Texas proved it. Requiring an autoclave in nail salons means nothing. Just more autoclaves are sold to perpetrate a false premise.

I am glad to report, however, that many salons are grabbing onto safety as their mantra. The technicians are trained to...
