OH Podiatrist Risks Career to Blow Whistle on Medical Fraud
 
Something didn't seem right to Dr. Barbara Petkovic. “I didn't like it,” the Northeast Ohio podiatrist recalled. “It made me feel very uncomfortable.” Patient after patient passed through. "I feel like it was an assembly line," she said. It soon became apparent: she was working amid an assembly line of false claims and illegal kickbacks. “I just felt it was wrong. I felt I was involved in it. The whole thing scared me," she said. Instead of continuing, she blew the whistle against her employer, Mobile Medical. After years of litigation, the company eventually paid a $4.5 million settlement to the federal government and Petkovic. Despite an FBI raid, another company, Provider Services (a nursing home chain), has so far walked away unscathed.

Federal prosecutors won't say why they've dropped the case. The company, which denies any wrongdoing, won't comment. But Dr. Petkovic refuses to drop her end of the suit and intends to pursue the case without the help of the federal government. "It cost her her job. It really damaged her career but she came forward anyway and tried to make things right,” said her attorney Warner Mendenhall of Akron.

Surgical podiatrist Kasandra Trott, DPM recently joined Geisinger-Lewistown Hospital. Dr. Trott received her medical degree from Rosalind Franklin University of Medicine and Science, William M. Scholl College of Podiatric Medicine, North Chicago, Illinois, in 2013.

Biomechanics Seminar at NYCPM Honors the Legacy of Richard O. Schuster, DPM 
 
The stated goal of the 2017 Richard O. Schuster, DPM Memorial Biomechanics Seminar at NYCPM in November was to re-establish biomechanics as the cornerstone for the practice of podiatric medicine and surgery based on the principles, research, and lifelong dedication of NYCPM alumnus, teacher, administrator, and benefactor Richard O. Schuster, DPM (’37).

The one-day seminar featured presentations by 10 leaders who informed the SRO audience of practitioners about cutting-edge biomechanics and their application to a range of pathologies and lower extremity function. In general, the annual seminar, now in its fifth year, provides a historical and clinical review of Schuster’s many contributions to the field of podiatric biomechanics. This “commercial-free” seminar was made possible by the generous donation of a patient of the seminar’s Scientific Chair, Joseph C. D’Amico, DPM.

FL Podiatrist Shoots Last Known Video of Lost Argentinean Submarine
 
This unedited video of the missing Argentinean submarine, the San Juan was taken on November 6th 2017 by podiatrist Burton S. Schuler, DPM. The San Juan sailed on November 8th 2017 with 44 crew members and has not been seen since. The video  was recorded in the port of Ushuaia which is part of Tierra del Fuego archipelago. Dr. Schuler was leaving on November 6th to be part of an expedition to the Antarctica Peninsula.

Schuler said, "the weather that day was beautiful, so I decided to get some video. After taping for two minutes, I saw something else in the water. When I asked,  a "local" about it, she said the name of the sub was the 'San Juan'. In spite of the fact that Argentina's navy detected some brief satellite calls on November 18th that may have come from the San Juan, it is still missing.

Takeaways on the 2018 MACRA Final Rule - Part 9
 
9. CMS isn't changing the Advanced APM qualification criteria. CMS will maintain the existing Advanced APM qualification criteria through the 2019 and 2020 performance years. Risk-based programs eligible for the Advanced APM track still will require a revenue-based nominal amount standard at 8 percent of the estimated average total Parts A and B revenue of eligible clinicians.

9. Establish Automated Payment Plans
 
"Negotiating a payment plan with a patient is a good first step, but then the practice has to send a statement again every month and hope the patient complies. Historically, this has resulted in [sporadic] payments that yield less than 50 percent of the expected reimbursement. [Keep the patient's] credit card [on file] and get authorization to submit payment every month or get their bank account information and do a monthly ACH. Automating the [payment plan] results in the near doubling of collections - Sean McSweeney, founder and president of Apache Health, a medical billing company in Marina del Rey, CA.

What is the current standard of care in podiatry with regard to use of electrical drills for dystrophic toenails? Are podiatrists still doing this? If so, are the drill bits individually sterilized and used only once? Is the use of drills to debride dystrophic toenails still taught in the podiatry schools?

I worked for Aggeus for a short time back in 2011. After a few months, I was told by the owner, Zev Gray, that I was “low man on the totem pole in billing.” When I responded that all I was doing was routine care, Gray gave me recommendations on how to up-charge. “Squeeze the back of the foot and if the patient jumps, you can bill for a posterior tibial tendinitis”. I replied that most [patients] were in wheelchairs and were being seen for routine care. Gray got "personal", and I quit. The moral of the story is to be true to your profession. Zev, his partner, and his wife have all had their day of reckoning.

When asked what was his chief complaint, the patient responded that he had a severe case of "Morton's Neurosis".  After speaking with him for a while, I realized he was right. There certainly was a neurosis there as well.
