PODIATRISTS IN THE NEWS
Most Foot and Ankle Problems Do Not Require Surgery: WI Podiatrist

"Most foot problems do not result in surgery," says Mitch Hulbert, DPM, from Columbia St. Mary's, part of Ascension. "There are always other options that have to be catered to the patient. What are their goals, what are there levels of physical activity? Are you an elite athlete, or do you just want to walk without pain?"

Dr. Mitch Hulbert

"Arthritis, for example, is a common problem. All the joints in the foot and ankle are weight-bearing joints, so they have to take the stress of everyday life," says Dr. Hulbert. Your feet must last a lifetime, and most Americans log an amazing 75,000 miles on their feet by the time they reach age 50. "There are a lot of ways to treat arthritis, many of them not painful."

PODIATRISTS IN THE COMMUNITY
GA Podiatrist Thankful for Hero who Saved His Mom From CA Wildfire

Mario Monte, a nursing student, was honored Tuesday as a hero in saving seniors from a raging wildfire last month. No one knows that more than Dr. Ed Lopez, the son of Irene Lopez, 86, whose abandoned red wheelchair that Monte had used to transport her into his car was captured in stark relief in the ashes days later by photographer Karl Mondon of Bay Area News Group. “I have a lot to be thankful for this Thanksgiving,” said Dr. Lopez, a Georgia podiatrist, in a phone interview. “If she (his mother) had died in that fire, I can only wonder how awful that would have been.”

Dr. Ed Lopez and his Mom's Burned Wheelchair

Lopez, who is flying back and forth across the country every week to check on his mom at her new senior home in Santa Rosa, said he offered to pay for Monte’s school textbooks, but the nursing student declined. He said the dean (at the College of Marin) took him down to the school bookstore and said, ‘You can have any books you want,’” Lopez said. He remains grateful to Monte.

PODIATRISTS AND FOOTWEAR
NJ Podiatrist Discusses Importance of Supportive Footwear

Wayne Feldman, DPM says that standing for long periods of time puts us at greater risk for certain conditions of the foot, such as plantar fasciitis, soreness, strain, and leg pain.

Dr. Wayne Feldman

In terms of shoes, Dr. Feldman recommends a supportive, structured running sneaker, such as those offered by Asics and New Balance. He does not recommend lightweight sneakers, such as Nike minimalist sneakers, which are very bendable, and compares them to standing barefoot. He recommends a slight heel elevation, saying that shoes should not be too flat.

MACRA UPDATE
Takeaways on the 2018 MACRA Final Rule - Part 9

9. CMS isn't changing the Advanced APM qualification criteria. CMS will maintain the existing Advanced APM qualification criteria through the 2019 and 2020 performance years. Risk-based programs eligible for the Advanced APM track still will require a revenue-based nominal amount standard at 8 percent of the estimated average total Parts A and B revenue of eligible clinicians.

PRACTICE MANAGEMENT TIP OF THE DAY

Tips to Improve Patient Collections - Part 7

7. Conduct ongoing staff training and scripting

"There should be an awareness of these high-deductible plans' [as well as] scripts to help the staff educate the patients. Some of the basic scripts, of course, are not asking if the patient wants to make a payment. Instead, they should ask how the patient wants to make the payment." - Marie Debs, MBA

CODINGLINE CORNER
Query: Patient Seen in Office While Residing in SNF

We have a patient who was seen in the office for a post-operative visit. The patient had resided in a skilled nursing facility (SNF) for two weeks at the time of the visit. His wife brought him to the visit. We took an x-ray in the office, billed Novitis Medicare, CPT 73630 (1 unit). Medicare is saying they overpaid the claim and want to take back what they paid. The reason given: “Services within a Skilled Nursing Facility are subject to consolidated billing”. The representative is telling me we have to bill the nursing facility, they bill Medicare and then pay us. She cites the 2017 CMS site for guidelines on Consolidated Billing. CPT 73630 is not on this list of procedures that can be billed by the doctor’s office. This just doesn’t make sense. Has anyone else run into this?

Peter Cuesta, DPM, Ocean Pines, MD

Response: The rep was only partially right. They inappropriately only overpaid you for the technical portion of the x-ray study allowed. X-ray studies (the technical component) fall under Consolidated Billing, but you can bill and get paid for the x-ray study professional component to your Medicare Part B contractor (Novitas) by applying a “26” to the x-ray code.

The skilled nursing facility (SNF) is paid in a lump sum for services such as the x-ray study technical component. If you have a prior arrangement with the SNF, you may get paid directly by the SNF for the technical component. If you haven’t set up an agreement with the SNF, contact their administration. The worst that can happen is that you won’t get that portion of the study…but you’ll know for the future.

RESPONSES/COMMENTS (NEWS STORIES)
From: Robert Spalding, DPM

Many states do not allow you to bring your own instruments and that would be in violation of some state regulations. Many times, the instruments brought from home are shared with others in the family who have a nail infection, and are not properly sterilized. Storing contaminated personal instruments that could cross-contaminate other instruments in a salon is no answer either.

However, a better protocol has emerged. The Boston public health department has mandated new standards by implementing autoclave sterilization for salons above and beyond what the Massachusetts cosmetology board requires. Boston for the last 2 years requires...
