NY Podiatrist Urges Those Using Nail Salons to Bring Their Own Instruments

Dr. Bruce Pinker, a New York podiatrist, urges those seeking pedicures to be careful. “Many patients believe they became infected with toenail fungus at the nail salon,” said Pinker, who specializes in foot and ankle sports medicine, diabetic foot care, and foot laser treatment. “Interestingly, it is usually fungus that causes discoloration of the toenail, but also yeast and mold may be the culprit.”

Dr. Bruce Pinker

When someone really likes their pedicurist but is a bit uncertain about the salon, Pinker suggests they buy their own pedicure instruments at a beauty salon. Take them home, scrub them and put them in a dishwasher that has heated drying. Then wrap them in cellophane and take them to the nail salon.

IN Podiatrists Collect Socks for Homeless Vets

Indiana Podiatry Group is accepting new or gently used socks for homeless veterans around central Indiana from now until Dec. 31 for its third annual Sock it to IPG sock collection drive. At the end of each year, the organization conducts the sock collection drive as a way to give back to the Hoosiers who served our country.

The goal is to collect as many socks as possible to donate to homeless veterans and their families in and around central Indiana. Last year, the group collected more than 2,800 pairs of socks. All donations are presented to the Hoosier Veterans Assistance Foundation, an Indianapolis-based nonprofit organization dedicated to helping homeless veterans find jobs and providing them with food, shelter, and necessary items.

MEDICARE NEWS

Improper Medicare Payments Drop by Nearly $5 Billion

The CMS on Tuesday unveiled that the number of improper payments doled out by Medicare is the lowest it has been since 2013, accounting for less than 10% of overall Medicare payments. All in all, the CMS' estimated improper payments dropped $4.9 billion from 2016 to 2017, marking a rate decrease from 11% in 2016 to 9.5% in 2017.

Improper payments include fraudulent claims, payments distributed to the wrong recipient or for the wrong amount, payments with insufficient documentation, and issues where the recipient uses the funds improperly.

MACRA UPDATE
Takeaways on the 2018 MACRA Final Rule - Part 7

7. The final rule raises the performance bar to avoid payment penalties in MIPS slightly overall. In order to avoid a payment penalty in 2019, clinicians need to earn three points across the three MIPS categories in 2017. For payment adjustments in 2020, CMS is raising that threshold to 15 total points for the 2018 performance year. This is still a fairly low bar to clear and may dilute bonuses in 2020 for high performers..

PRACTICE MANAGEMENT TIP OF THE DAY

Tips to Improve Patient Collections - Part 5

5. "Give patients the option to enroll in eStatements so that balance information comes directly to their inboxes with a link to make an online payment in a few easy clicks. Enable patients to set up a digital wallet through your patient portal to store a payment method on file. Finally, set up text message notifications to [communicate with] patients. [This can also] help reduce inbound call volume." - Jeff Lin, senior vice president of product management at InstaMed, a healthcare payments network provider headquartered in Philadelphia.

CODINGLINE CORNER
Query: Billing Excision of a Heel Neuroma

What is the correct way to bill for the excision of heel neuroma?

K. Kyle Ballew, DPM, Waco, TX

Response: Since this is not a “Morton’s neuroma” (forefoot), CPT 28080 does not apply. I recommend using the more generic code, CPT 64782 (excision of neuroma, hand or foot, except digital nerve). This would be appropriate for the procedure you have described.
