Foot and Leg Pain May be Tied to Heart Disease: KY Podiatrist

Your foot and leg pain can be related to heart disease. Coronary artery disease is narrowing of the arteries in the heart where peripheral artery disease is narrowing of the arteries in the extremities and feet. PAD is very common in people with coronary artery disease," says podiatrist Sarah Voelkel, DPM. Hardened, narrowed arteries (atherosclerosis) are due to plaque buildup along the arterial wall. The plaque consists of fats, cholesterol, and other substances.

Dr. Sarah Voelkel

"When there is severe reduction in blood flow or complete blockage, there is tissue damage or death. In the heart, this is considered a heart attack. In the feet and legs, this can be limb threatening where you are at risk of toe, foot, or partial leg amputation," says Dr. Voelkel.

PODIATRISTS IN THE COMMUNITY
WA Podiatrist Donates Handmade Ceramic Bowls for Fundraiser

The public is invited to purchase handcrafted ceramic bowls and share a meal to raise money for families in need and to also support a capital campaign to build a new food bank. Dr. Colyn Nouv, a podiatrist who joined the Lake Stevens clinic a couple of months ago, is donating bowls she made in her garage studio in Snohomish. She took ceramics classes in high school. She picked up the hobby again in medical school.

Dr. Colyn Nouv

“As a doctor, it’s important to be creative to find solutions for patients seeking help,” she said. Ceramics taps into that same energy, as you try to see things in a different light, Nouv said. She’s made about 10 bowls for the fundraiser.

PODIATRISTS AND POLITICS
OH Podiatrist Favors Drug Price Relief Act on Nov. Ballot

The Ohio Drug Price Relief Act — Issue 2 on the November ballot — has caused plenty of debate. It would require that the state, including the Ohio Department of Medicaid, pay the same or lower prices for prescription drugs as the U.S. Department of Veterans Affairs — which currently negotiates drug prices at least 24 percent less than other agencies.

Dr. Tanisha Richmond

Dr. Tanisha Richmond, a Dayton-area podiatrist, is in favor of the proposal. “The pharmaceutical companies are against it and trying to make it negative… It’s a cost that’s paid for by the taxpayers, and ultimately we’re suffering.”

ON THE INTERNATIONAL LECTURE CIRCUIT
U.S. Podiatrists Speak at Icelandic Conference on Wounds

Drs. Lee Rogers (CA) and Animesh ("Andy") Bhatia (OH) spoke at the Kerecis Northern Lights Wound Conference in Reykjavik, Iceland on September 22. The conference highlighted the use of North Atlantic cod fish skin on chronic wounds.

(L-R) Drs. Lee Rogers and Animesh ("Andy") Bhatia

The Kerecis Omega3 skin replacement product has recently been approved by the FDA for use on acute and chronic wounds. It is rich in omega-3 fatty acids which are anti-inflammatory and are thought to have an effect on wound healing, especially those chronic wounds caught in the inflammatory phase of healing.

PRACTICE MANAGEMENT TIP OF THE DAY
Things to Know Before Switching to DPC - Part 8

Direct primary care (DPC) is a throwback to an earlier time, where physicians abandoned reliance on insurance in favor of a direct payment agreement with their patients. They may not be traveling to patients' homes, black bag in hand, but those embracing this approach are finding that, at least in part, it offers a welcome return to the medical practice of earlier times. Before making such a move, what are some of the factors to consider? Here are thoughts from physicians with first-hand experience.

8. Navigating Tricky Waters

"DPC is the new kid on the block: innovative, disruptive, and promising. Investigate it. While it is not necessarily the answer for everyone, it is certainly an answer for many. The legislative environment is a constantly shifting target. Opting out of Medicare is complicated. Ensuring that your DPC is following all the appropriate regulations is important. Migrating from a traditional medical insurance-based practice to DPC is a tough road to navigate. Having a 'blended practice' is tricky." - Ripley Hollister, MD, a primary-care physician in Colorado Springs, CO

QUERIES (CLINICAL)
Query: Pain in Left 2nd Toe Following Ingestion of Kit Kats

My patient is 69 year old with intermittent pain in the left second toe following consumption of several Kit Kats. He stated that he has eaten other chocolate products and has not had any similar reaction. His medical history includes Stage 3 chronic kidney disease, DM, and depression, but he is under treatment and control. His meds include Glipizide, hydrochlorothiazide, isosorbide, Losartan, and indomethacin. His labs are WNL.

The first episode occurred a few months ago with a duration lasting 1-2 days followed by complete resolution.  I just thought this was a curious finding. I had never heard of anything like this. There are no biomechanical or LE issues and his regular care is for onychocryptosis. Has anyone ever seen this idiosyncractic reaction?

Editor's note: The ingredients label of this product says that it contains  PGPR (Palsgaard4150) (an emulsifier). The FDA has deemed PGPR safe for humans as long as you restrict your intake to 7.5 milligrams per kilogram of body weight.
