AT THE COLLEGES
NYCPM/FCNY Receive Gift of State-of-the-Art Technology

Thanks to a generous donation by 20/20 Imaging, The New York College of Podiatric Medicine (NYCPM) and the Foot Center of New York (FCNY) have acquired the updated technology of the company’s state-of-the-art HS1 ultrasound scanner. The scanner is already seeing heavy use, in both clinical applications with patients in the Foot Center of New York and for education and training purposes in NYCPM. In the FCNY, the scanner is used for both diagnostic imaging and ultrasound-guided injections among other uses.

Drs. Marty Wendelken, Randy Cohen, and Stephanie Maurais with the new HS1 ultrasound scanner.

According to Dr. Michael J. Trepal, Vice President for Academic Affairs and Dean at NYCPM, “going forward, the HS1 scanner will enable our clinical staff to provide improved care for our patients and enhanced education for our residents and students. We are extremely grateful to 20/20 Imaging for its support of podiatric medical education.”

PODIATRISTS IN ACADEMIA
CA Podiatrist Joins Keck Medicine

Every 20 seconds, someone in the world loses a limb to diabetes. David G. Armstrong, DPM, MD, PhD, is working to change that. Armstrong, professor of clinical surgery at the Keck School of Medicine of USC, joined the faculty in August. He is an internationally lauded diabetic wound-care expert, the first podiatric surgeon to join the Society for Vascular Surgery and the Royal College of Physicians and Surgeons of Glasgow, and the youngest person ever inducted into the PM Podiatry Hall of Fame.

Dr. David Armstrong

Armstrong is the founder and co-director of the Southwestern Academic Limb Salvage Alliance (SALSA), an organization that already operates at colleges and universities in more than a dozen countries. Part of his mission is to expand the organization further, with Keck Medicine now the primary hub. Over the next generation, the team hopes to reduce and eventually eliminate preventable diabetes-related amputations.

PODIATRISTS AND FOOTWEAR
Flats Not Necessarily Better for Feet: FL Podiatrist

You’d think a flat automatically is healthier for your feet and better for walking compared to other styles, but you’d be wrong. In fact, a flat with arch support, by definition, may be a bit of a misnomer, says Pensacola, Florida podiatrist Grace Torres-Hodges, DPM, a spokesperson for the American Podiatric Medical Association. A true flat will likely not have an arch support, which is why she recommends choosing a “flat” with a half inch of a heel (ideally, a wedge), which will support your foot and your arch.

Dr. Grace Torres-Hodges

You also want to look for a heel counter—the firm part of your shoe that wraps around the back and keeps your foot secure—that has some heft. Squeeze it to be sure. A “floppy” flat that has no structure won’t do anything for your comfort, even if you love the style.

HEALTHCARE LEGISLATION
Bipartisan Effort to Stabilize Insurance Market Hits Snag Over State Flexibility

The senior Republican and Democrat on the Senate health committee courteously clashed Tuesday over how much freedom Congress should give states to modify the Affordable Care Act's benefit rules in order to make individual coverage more affordable. Most of the senators and expert witnesses speaking in the third of four planned hearings agreed Congress should swiftly fund payments to insurers for the ACA's cost-sharing reductions for lower-income enrollees, though it wasn't clear that Republicans supported insurers' calls for long-term funding.

Disagreement over how much flexibility to give states to redesign the ACA's coverage requirements under state innovation waivers could doom prospects for passage of a stabilization package before the Sept. 27 deadline for insurers to decide whether to offer individual-market plans in 2018 and what their premiums will be. Without certainty about CSR payments, insurers are expected to raise rates by as much as 20%, or stop offering coverage on the exchanges entirely.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Habits of High-Performing Medical Practices - Part 4

We ran a survey in March 2017 and found that 24 percent of practices qualified as "high performers" who were seeing a lot of growth and success. About 15 percent of practices qualified as "falling behind." What was causing some practices to thrive and others to fail? I found a few surprising correlations:

4. Planning Ahead: High-performing practices are almost twice as likely to have a plan in place for the transition to value-based care and also almost twice as likely to be ready to earn a MACRA incentive this year compared to falling behind practices. Understanding the opportunities in this changing healthcare economy is crucial to your practice success.

RESPONSES/COMMENTS (CLINICAL)
From: Joon Yim, MD

I completely disagree with the comments of Dr. Chaskin. Alcian Blue is not a conventional stain used for diagnosing fungi. If anything, GMS is a more standard stain than Alcian Blue. But using PAS with another stain does not make the diagnosis any more accurate than simply repeating PAS. The shortcoming in this kind of testing is not the stain itself, but the fact that in each section we stain, we may not be cutting the diagnostic area of the sample. So, the more sections we perform, the more likely we will make the right diagnosis. On the other hand, PCR will be much more accurate, although more expensive.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Bromhidrosis Detector of the Day

People too shy to tell someone that their feet stink really bad can now rely on a robot dog to do the job. Meet Hana-chan, a 15-cm (6-in) robot from Japan’s Manufacturers Next Technology that barks at the scent of stinky feet — and collapses when it can’t handle the foul stench.

Japan’s cute new robot pup faints if it catches a whiff of stinky feet.

The robot gets its clever name from hana, which means “nose,” and is a common nickname for girls in Japan. Hana-chan works through an odor detection sensor, and when things are unbearable, sprays air-freshener to solve the problem.
