FL Podiatrist Discusses Diabetic Neuropathy
 
“Are you having any numbness, tingling, or burning in your feet?” They’re common questions, Dr. Chelsea Viola, a podiatric surgeon on the medical staff of Lee Health, asks patients who are living with diabetes. “The biggest thing with diabetes is a lot of patients end up getting nerve pain. They also end up getting difficulties with circulation. These two things are very related to diabetes.”

If a patient develops burning, tingling, or numbness in their feet, it’s time to make an appointment with a podiatrist. “There are some things we can do to help the symptoms. Reversal of nerve damage is very difficult. Once you get to the numbness, there’s really no reversing that,” she said.

“There is a genetic component for problems such as bunions. If these run in your family, it’s very important to have an evaluation by your podiatrist early," says podiatrist Jacqueline Sutera, DPM. Setting aside this majorly unfair advantage, podiatrists do recommend a few simple steps to combat the negative effects.

Most of us are too busy burning calories with cardio exercise or building our six-packs to notice our feet, says consultant podiatrist Matt Fitzpatrick at the College of Podiatry. "We spring out of bed and just expect them to kick into life every day and get us from A to B," he says. "But feet are very clever and complex in the way they are articulated, and they’re just as important to exercise as your biceps or hamstrings — in fact, even more important because they never get to rest."

A recent Randstad U.S. survey found both clinical and non-clinical healthcare employees are eager to have the option to occasionally work from home. While working remotely may not be feasible for some roles, offering greater flexibility around scheduling can help alleviate fatigue and improve work-life balance

Query: Medicare Advantage Audited by Medicare?
 
There is always concern when billing Medicare that -25 modifiers, at-risk foot care codes, and -59 modifiers can be red flags for audits. I am extra careful to include photos of pathology in my EHR notes to protect myself. How about for PPO medicare advantage plans where I am not an in network provider? Does this billing data go to medicare and affect my audit risk? Can Medicare audit me for these PPO patients, or are these audits done by the commercial carriers?

Response: When you provide services for patients with a Medicare Advantage plan (such as Humana), any audits would be conducted by the Medicare Advantage carrier, for example Humana. Medicare (CMS) does not track individual providers within Medicare Advantage plans; to the extent that is done, it is by the Medicare Advantage carrier itself. These plans are run in a very similar fashion to commercial insurance plans although they are required to follow certain Medicare standards. Even if you are not a network provider, the carrier can audit you and request your records–you implicitly agree to this when you submit a claim for reimbursement.

QUERIES (NON-CLINICAL)
Query: Disposable Instrument Kits  
 
I had a rep in the office earlier peddling disposable surgical trays; specifically, one kit that was a nail nipper and an emery board. The other kit he demonstrated was a P&A kit - with an English anvil type nail splitter, elevator, beaver handle, and file. These were fully disposable kits and appeared to be of decent quality. They also are selling a "wound debridement kit". The nail kit was $10. The P&A kit was about $20.
 
My question is this: is autoclave sterilization "not good enough" anymore? He said that medicine was going to all disposable instrumentation eventually and that fungal spores could survive steam sterilization. I did quick math and figured if I paid $10 for a kit for every nail debridement, and $20 for every nail procedure, my profit margin would really suffer!

Dr. Udell’s question is an important one. Patients with diabetes, PAD, chronic edema, history of recurrent cellulitis, or who are immune-compromised, require active treatment for onychomycosis. The high cost of topical antifungal medications reinforces the need for proper speciation to assure the possibility of response to the prescribed agent. Utilization of topical antifungals should be restricted to early, distal disease. 
 
Personally, I utilize topical amphotericin B compounded with urea, terbinafine, and thymol  (available through EBM), combined with nail reduction for topical management. Oral terbinafine, when appropriate, remains the most effective therapy for management in the adult and geriatric patient. It is the most cost-effective therapy available. The potential adverse sequella of oral terbinafine has been exaggerated by those selling alternative topical agents, or simply not understood by those unfamiliar with the literature on this subject.

When someone is recognized by a single name, that sort of says it all! Lowell, Congrats and best wishes on your well-earned retirement. You certainly were my podiatric mentor and have been my close friend for 47 years. I'd be remiss if I didn't remind you and 98 of my classmates from OCPM '70 of the weekend in 1969 when Marc Lenet and I begged you to come to Cleveland. Our class chipped in and hosted you for a long weekend of podiatric biomechanical and surgical teaching that until then had been somewhat lacking. 
 
You gave all of us the motivation and pride to complete our education and proudly become podiatrists and podiatric surgeons. On behalf of the entire OCPM Class of 1970 (and the Northlake residents of 1971), I thank you for the truly tireless, professional, and self-sacrificing time and knowledge that you have shared with all podiatrists. Come on down to N. Florida and play golf with me this winter.
