The Fury of Irma Floods Florida
 
As we all know, Florida is being pummeled by Hurricane Irma. Unfortunately, this will affect many podiatrists as well as the general population of the entire area. Our thoughts and prayers go out to the people of Florida and other locations affected by Hurricane Irma and its aftermath. We extend a platform for any podiatrist affected by the storm. As with Hurricane Harvey, the podiatric community stands ready to provide assistance to those affected.

REMEMBERING 9/11

Editor’s note: On this, the 16th anniversary of 9/11, we continue our proud tradition of publishing the following note from the editor, which was published in PM News on 9/12/01 and circulated via the Internet throughout the world.

It is just after 3 AM here in New York City and I simply can't sleep. Looking out the window of my bedroom on the 17th floor, there is an unobstructed view of the Manhattan skyline. I search again for the once familiar sight of the two tall white buildings that marked the lower end of the skyscraper-filled island. All I see are some eerie search lights of the rescue crews.

Like everyone else, I have been in shock all day, particularly since I witnessed in real-time the morbid views of the second plane hitting the World Trade Center and the sequential collapse of both towers.

The gamut of emotions runs from shock, fear, sadness, to anger. Barely one half-century after the Holocaust, despite all our technological advances, man remains uncivilized. The realization sets in that the terrorists who committed this crime could have and still could set off a nuclear bomb, killing millions. The safety and security we have taken for granted in America is and will never be the same.

My phone has been ringing all day. Family and friends call to determine that everyone is safe. My mom calls to tell me my brother was scheduled to be at the WTC at 9 AM and was at the subway station when the first plane hit. Had the plane hit five minutes later, he would have been on an elevator headed for certain death.

Unfortunately, many thousands were not so lucky, including many employees of Blue Cross/Blue Shield, which only a few years ago moved from midtown to this location. There will be many tears shed as the names of those murdered in this horrific disaster become known, many unnecessary funerals to attend. So many innocent lives lost for no reason. So many families destroyed.

Later this morning, my wife and I will donate blood and attempt to do what we can in this time of crisis. We know the entire podiatric community will collectively pray for those affected by this catastrophe and provide whatever support is needed.

Albert Musella, DPM is a foot doctor and tech geek who founded what became the non-profit Musella Foundation for Brain Tumor Research & Information before his sister-in-law died of GBM. She was diagnosed in 1992. He discovered there was no easy way to search out current and experimental treatments.

Musella used, no kidding, the first major commercial online service provider, CompuServe, to start his process in the early ’90s. He set up an online support group and organized information that patients and their doctors could grab and use, not that there were great choices. But his sister-in-law lasted an unusual eight years.

We ran a survey in March 2017 and found that 24 percent of practices qualified as "high performers" who were seeing a lot of growth and success. About 15 percent of practices qualified as "falling behind." What was causing some practices to thrive and others to fail? I found a few surprising correlations:
 
2. Realism: Compared to falling behind practices. I'm not sure if this is a result of high performers being better prepared (many have a plan for value-based care transitions) or if their concern reflects a more realistic assessment of the challenges facing practices today. So, you're in good shape if your physicians are satisfied in their careers but also worried about the future.

Query: Lymphedema and Filariasis
 
My patent is a 53 year old white male, paraplegic from MVA 10 years ago. He has a 4-year history of swelling and deformity of his toes bilaterally. He lived in the South American jungles from 1996 to 2005 doing surveillance/DEA type work.
 
Lymphedema and Filariasis
 
His legs are both swollen with a history of DVT, and he is on warfarin. I came up with a diagnosis of lymphedema and filariasis. He has had no treatment for this condition. I  am looking for additional information about diagnosis and treatment.

RE: Alcian Blue Stains as an Alternative to Genetic Testing

PCR assays or genetic testing are more expensive than Alcian Blue Stains. Many podiatrists do not order genetic testing because of cost. Alcian Blue Stains may not be as expensive and may with PAS yield a higher positive result. Alcian Blue stains combined with PAS is likely to result in more positive onychomycotic results. Thus, if you are concerned about cost and do not want to order genetic testing, consider Alcian Blue Stains. If cost is not a factor, my personal opinion is that genetic testing is the optimum choice for detecting onychomycosis.

I live in Houston and am dealing firsthand with the disaster there  I'm helping podiatrists and their staffs where I can; whether it's helping to gut out their homes/offices, driving around collecting supplies, gathering demo teams, and/or walking them through the process of getting their lives back together. It is overwhelming because 73% of the podiatric physicians I service in the South Texas region have been affected!
