AT THE COLLEGES
TUSPM Continues Proud Tradition at Boston Marathon
 
Keeping a proud tradition once again, third and fourth year students from the Temple University School of Podiatric Medicine (TUSPM) served as part of the podiatric medical team under the direction of team captain Jonathan Kaplan, DPM. The TUSPM team was led by director of sports medicine, Howard J Palamarchuk, DPM and student sports medicine club president Emily Keeter.

The team of 11 dealt with a warm day that produced many foot and ankle problems related to heat and overuse. Along with attendings and residents from the greater Boston area, the team was well received by the runners needing care. This experience offered a nice end experience in sports medicine for the senior students who will graduate three weeks from today.

Repetitive Trauma Most Common Cause of Black Toenail: NY Podiatrist
 
The most common culprit for black nails, repetitive trauma can result from running, or from wearing any type of ill-fitting footwear. If a black nail crops up shortly after a workout or a day spent in too-tight or too-loose shoes, this is likely the cause. Repetitive trauma ranges from mild (i.e. a small, painless, black-and-blue discoloration beneath the nail), to severe (i.e. large, bloody blisters between your nail and nail plate), explains Jacqueline Sutera, DPM and spokesperson for the American Podiatric Medical Association.

In mild cases, no treatment is needed and the black nail will simply grow out. In severe cases, beneath-the-nail blisters can cause the nail to detach—either partially or fully—from the nail plate.

NJ Podiatrist Gets 27 Months in Prison in Bribery Case
 
The former mayor of Passaic, Dr. Alex D. Blanco, was sentenced Tuesday to 27 months in prison with three years of supervised relief for soliciting $110,000 in bribes from two developers hired by the city to build affordable housing on Paulison Avenue. The former mayor must also make $110,000 in restitution to the two developers involved in the case. The sentence is below the federal guidelines and Blanco faced up to 10 years in jail. Blanco's attorney, Joseph Hayden Jr. requested leniency from Martini, citing Blanco's 13 years of public service as a school board trustee and as mayor, lack of criminal record, and his service as a podiatrist at the Veteran's Affairs hospital in East Orange.

Query: Protocol for Testing When Prescribing Oral Antifungals 
 
I have been doing KOH, cultures, and liver function tests when prescribing oral antifungals for many years. Most of the time, these tests have not revealed much useful information.  As we all know, KOH and cultures are fraught with errors (please do not tell me to use other tests. I work at a community health center and my patients can’t get other tests covered by their insurance). The hepatic function tests rarely indicate serious liver problems. My question is this: has there been any change in standards of care in the testing required to treat onychomycosis prior to initiation of treatment by oral medications?

There is only one source for MACRA information and that is qpp.cms.gov. This is the CMS website for MIPS. On this website, you will find that there is a Clinical Practice Improvement Activity called, “Implementation of Fall Screening and Assessment Programs”. There are also three Quality measures dealing with falls. I think the one you are asking about is the Quality measure #154, “Falls: Risk Assessment". On the QPP website, you can read every detail about this measure, the denominator and numerator, as well as the codes for reporting.

This is not such a great solution. Believe it or not, there are still a lot of people who don't have credit cards at all! This includes many older potential patients who don't trust those newfangled things. There are also many individuals who, for various reasons, don't qualify for a card but who would otherwise be excellent, deserving patients. Shouldn't these people have access to quality footcare too?
