IN Podiatrist's Forensic Gait Analysis Leads to Arrest in Robbery Case

A small, rural sheriff's department in Tennessee made an arrest in a robbery case with forensic podiatry, analyzing how a person walks, for the first time in the U.S. It all started more than a year ago when three masked men walked into Barry’s One Stop Package Store near the Tennessee-Alabama line in Wayne County, TN. “They had masks, gloves,” said Wayne County Sheriff Ric Wilson. "There were no fingerprints, no witnesses. They were in and out in one minute and 38 seconds." All the department could do was go back to the surveillance tape from the days prior to the robbery and try to see if anyone was casing the place.

Dr. Michael Nirenberg

Wayne County sent the robbery video and the video of the men lingering at the cooler to forensic podiatrist Dr. Mike Nirenberg in Indiana. He did a detailed analysis and identified the robber as a 100 percent walking match to the man lingering at the cooler, days earlier. The sheriff brought the evidence to District Attorney Brent Cooper. “The doctor does a great job how he studies gait," Cooper said. "It was a great report. It is really compelling proof."

PRACTICE MANAGEMENT TIP OF THE DAY
What You Need to Know About Office-Based Surgery Laws - Part 2

In many states, an office-based surgery (OBS) medical practice is exempt from licensure requirements that apply to other surgical facilities, such as hospitals or ambulatory surgical centers (ASCs). Unlike these larger institutional surgical facilities, OBS practices do not have to satisfy the more stringent physical plant, safety and sanitary requirements imposed on the licensed facilities. Accordingly, an OBS medical practice can be located in any structure or suite of rooms established by one or more physicians for the private practice of medicine, but is typically limited to one surgical operating room.

OBS practices are generally subject to less stringent requirements because OBS procedures are relatively low risk and do not require the full complement of emergency room equipment and staff on hand in case of patient emergencies, which are mandatory in hospitals and ASCs. Receiving treatment in the OBS setting may be more efficient and convenient for the patient, especially where risk of an adverse outcome is low, and an overnight stay or a longer recovery time is unnecessary.

RESPONSES/COMMENTS (OBITUARIES) - PART 1A
RE: The Passing of Harry Goldsmith, DPM

I was very saddened to hear of the untimely passing of Dr. Harry Goldsmith, whom I have considered a friend for almost 25 years. Although I did not always agree with his rigid interpretations of the CPT code book, his true mission was to help all of us be very familiar with the rules and keep us out of trouble.

When I had the rare opportunity 25 years ago to become incorporated into a hospital-based major academic orthopedic surgery group, Harry was absolutely instrumental in guiding me through the negotiation process, advising me on issues I never even considered, resulting not only in successful negotiation, but a glorious career to follow. I never forgot those discussions and how emboldening and supportive he was. May he rest in peace.

RESPONSES/COMMENTS (OBITUARIES) - PART 1B
The Passing of Harry Goldsmith, DPM
From: Paul Kesselman, DPM

Words cannot begin to express my heartfelt deep sense of loss over the passing of my colleague, mentor, adviser, and most importantly a dear close personal friend. Despite his involvement in many projects, he was never too busy or too far away, to provide friendship, understanding, and guidance to me and many in our profession.

While the loss to our profession cannot begin to be measured, the loss to his wife, sons, grandchildren, and extended family is unfathomable. Harry did everything first class. He made it a point of...

RESPONSES/COMMENTS (OBITUARIES) - PART 1C
RE: The Passing of Harry Goldsmith, DPM
From: Mark S Block, DPM

I literally write this with tears in my eyes. I was and am still in a state of shock to hear that my friend and colleague has passed on. It still seems surreal. Some of us have had the opportunity and privilege to work closely with and get to know Harry Goldsmith. For me, he was not only a respected colleague but a friend. I have always looked forward to seeing and working with him on mutual issues. He had always been someone I admired and whose opinion and knowledge I respected.

Harry would always attack issues with 100+% drive. He was an advocate for the profession and respected as an authority. I will miss his insight into challenging issues. I will miss our frequent calls and discussions. I will miss working with him. I will miss his witty sense of humor. I will miss many things about Harry Goldsmith. I can go on for pages but those who knew Harry as I did understand.

My heartfelt sympathy goes out to his family. He will always have a place in my heart. He will never be forgotten. Harry, I will miss you but never forget you.

RESPONSES/COMMENTS (OBITUARIES) - PART 1D
The Passing of Harry Goldsmith, DPM
From: Joseph Borreggine, DPM

Harry Goldsmith was and will continue be the face of podiatry when it comes to the integrity of this profession. It's a sad day with his loss not only to this profession, but to his family as well.

He was the “real thing” that any person strives to be. With his great wisdom and wit, he was able to astutely provide podiatry the ability to understand coding in ways that no other profession could. He was able to provide sound expert advice in all things coding. His immense career in this profession cannot be put into words other than to say that he was a true...

RESPONSES/COMMENTS (DME)
From: Stephen Musser, DPM

They look at/for the following...

1. Must be a physical location, not a PO Box.
2. Shoes/products must be displayed or visible for patients to see.
3. Supplier standards must be posted in DME room.
4. Hours of operation posted at the place of business.

RESPONSES/COMMENTS (NON-CLINICAL)
RE: Formula for Value of a Practice
From: Jon Purdy, DPM

Practices are so individual there is no way to make generalizations about pricing, nor is it possible to closely estimate any particular practice in this forum. One either knows how to place value or doesn’t, in which case professional help is needed. There are tangible and intangible (so called goodwill) factors in pricing. Real estate aside, the tangible is the value of the equipment.

One intangible which is the most difficult to measure and possibly the most important, is practitioner performance. Is the selling doctor a dynamic personality who attracts and maintains patients? Is the seller practicing all aspects of podiatry, and therefore tapping into an entire patient population? Do you possess those same qualities and skills? No matter what the end value of a practice, if you do not possess the personality and skills to run a practice equivalent to, or better than the selling doctor, purchase of a practice may not be in your best interest.

Other considerations in practice valuation are: area demographics, provider saturation, payer mix, referral streams, patient turnover, and assessment of seller practice management skills. Each one of the aforementioned have significant in-depth evaluation measures.
