NY Podiatrist Discusses Gout
 
Gout is a form of arthritis. “It could be hereditary, or that your kidneys aren’t functioning correctly and you don’t have a certain enzyme to break down the uric acids found in meats, heavy sauces, and seafoods,” says podiatrist Hillary Brenner, DPM. “What happens is those urate crystals harden and go to the coldest part of the body—typically the big toe joint.” Your joint will look red, feel hot, and swell, and be super-sensitive to touch.

Since those things can also be signs of an infection, you should see a doctor ASAP. A podiatrist can help with the flare-up in your foot, but seeing an internal medicine doctor could help get to the root of the problem.

PODIATRISTS AND DIABETES
CO Podiatrist Participates in Stem Cell Clinical Trial
 
Diabetic patients participating in a national stem-cell study through Animas Foot and Ankle saw lingering and painful wounds on their feet finally heal. Nicholas Hugentobler, DPM, a podiatrist, enrolled 13 patients in a national study through the company Celgene. The trial is testing the effectiveness of injections made with stem cells from placentas that helped heal wounds, known as diabetic foot ulcers, said Dr. Hugentobler, whose clinic is affiliated with Animas Surgical Hospital.

The trial started a year and a half ago, and it will end in six months. When it is complete, Hugentobler will find out who received the injections. Hugentobler was skeptical of participating in clinical trials before working with the stem-cell injections because he didn’t fully understand the trials and the benefit to patients. But now, he plans to participate in more.
“This has been an eye-opening event for me,” he said.

If you have a post-surgical incision that dehisces, what is the ICD-10 code to use? I dispensed a tall CAM boot at this visit, since I could not put a cast on. I’m not sure what code to use for that as well.

Response: There are various ways to report dehiscence. These two are the most specific:
 
T81.31-  (dehiscence of surgical site, skin/subcutaneous tissue)
T81.32-  (dehiscence (internal structures) of surgical site)
 
Requires a 7th characters for either.
 
The CAM walking boot is outside of the global consideration and not impacted by global reductions in cost.  The HCPCS codes that can be billed (typically) for include:
 
L4360  (walking boot, pneumatic and/or vacuum, with or without joints, with or without interface material, prefabricated item that has been trimmed, bent, molded, assembled, or otherwise customized to fit a specific patient by an individual with expertise)
L4361 (walking boot, pneumatic and/or vacuum, with or without joints, with or without interface material, prefabricated, off-the-shelf)

Amen to Dr. Bryan Markinson's observation that doctors were blamed for under-treating pain just about a decade ago. In fact, in my residency and in our hospital (2004-2007), we were instructed that pain was "the 5th vital sign". 
 
Now, we are being blamed by opportunistic politicians as the cause of the opioid crisis. In Ohio, opioid prescriptions are now limited to 7 days, and additional orders require charted justification. Any of you ever see an ankle fracture heal in 7 days? I know my own didn't.
 
For months, I have felt like a lone voice for our position on this. The colleagues with whom I have spoken seem to be resigned to accepting the situation. It is re-assuring to hear such a respected spokesman in our field defend us against such unreasonable accusations.
