IN Podiatrist Finds Perfect Fit With Virtual Assistant
 
It wasn’t working for Dr. Timothy Barry. The Jasper podiatrist was spending hours and hours each day taking patient notes and transferring them into his office’s computer system — an extra load that forced him to stay late at night and also took away from the amount of direct attention he could give patients. And then, through the magic of the Internet, he met Joanna Fajardo, a virtual scribe. Residing in Bulacan, a province of the Philippines, Fajardo beams her face and services 8,000 miles across the Pacific Ocean to an iPad in Barry’s Jasper office Monday through Friday via video calling software.

The iPad is almost always in Barry’s hand or positioned in a manner that gives Fajardo a front-row seat to listen and take notes, after which she immediately uploads her work into the doctor’s electronic health record system. Barry said the cost of hiring Fajardo is about half of the expenses of staffing a traditional assistant at his practice.

Loss of Community Hospital Will Affect Welfare of the Community: AL Podiatrist
 
Elmore Community Hospital sits on a ridge just outside downtown Wetumpka. Sliding glass doors lead to a small entryway that sat empty on a recent Friday, except for two hospital volunteers in maroon vests. The hospital, which is licensed for almost 60 beds, usually fills fewer than 10 of them with patients who need overnight care. Like many rural hospitals - it has struggled financially as profit margins shrink and disappear.

For Dr. Tommy Garnett, a podiatrist preparing for surgery, the loss of rural hospitals such as the one in Elmore County could profoundly affect the welfare of the community. "This hospital is life-saving," he said. "Rural hospitals, basically it's the intake for the healthcare system."

FDA Permits Marketing of Device to Treat DFUs
 
On December 28th, 2017 the U.S. Food and Drug Administration permitted the marketing of the Dermapace System, the first shock wave device intended to treat diabetic foot ulcers (DFUs). The Dermapace System is intended to be used in the treatment of chronic, full-thickness diabetic foot ulcers with wound areas measuring no larger than 16 cm2 (about the size of a soda can top) which extend through the epidermis, dermis, tendon, or capsule, but without bone exposure. 
 
The Dermapace System is an external (extracorporeal) shock wave system that uses pulses of energy, similar to sound waves, to mechanically stimulate the wound. The device is intended for adult patients (22 years and older), presenting with diabetic foot ulcers lasting for more than 30 days, and should be used along with standard diabetic ulcer care.

Essential Provisions to Include in EHR Contracts  -  Part 7

As more healthcare providers are considering potential changes in electronic health records, it's time to take a fresh look at the contracts they are signing. Providers need to insert more protections in their legal documents as they make decisions to change vendors.

7. Require Access to Software Code
 
Agreements with healthcare IT vendors should also look ahead to the possibility than an EHR system change might happen in the future. In that case, it will be important for a provider to have access to the code for the vendor's product; that will be important for a provider to have access to data from the old system and effectively transition it to another system.

1. What is an MSO?
 
An MSO – sometimes referred to as a physician practice management company or an administrative services organization – generally refers to a business entity that provides non-clinical practice management services to a medical practice. Arrangements with MSOs typically involve the MSO performing most of the business and non-clinical functions of the medical practice, while the physicians remain in control of patient care and all clinical decision-making.

CPT 28296-T5 and was denied by Medicare. What am I doing wrong?

Response: CPT 28296 accepts bilateral billing for Medicare, so I suggest that you bill it with the “50” modifier appended.
 
You would bill CPT 28296-50, one unit, and double (or whatever you feel is appropriate) the price. It should then pay at 1.5 times the base fee schedule allowed.

Back in the '60s, I taught classes in roentgenology at what is now the Scholl College of Podiatric Medicine. I had read about (and believed possible) and subsequently taught my students that asymptomatic needles in the subcutaneous tissues discovered on x-ray could break apart and a fragment could penetrate a blood vessel and become a foreign object embolus -- possibly ending up in a vital organ. But to this day, I am not aware of this ever happening to anyone. Has any reader of PM News any information on this?

I just recertified last year for the second (and last) time—the Goldfarb Board Review Course was all I needed. I took the class over a weekend and then took the test the next week. Everything was fresh in my head and the test was not bad at all. As one of the instructors in the course said, "the recertification test is testing what you do every day."
